I've got an assignment over Thanksgiving that I need a little help with. It's not hard and would appreciate some help from my friends here!!

I need some vital signs from people I know. What it involves is this:

That's it!! If you could help me out it would be greatly appreciated!!

I like APA better than my last BP.

just remember-

- keep that title page tidy. title, etc, centered side to side and top to bottom.

-headings ARE centered, not underlined

-SUBheadings are not centered, but ARE underlined.

-doublespace everything.
