help..APA patient record format..due tomorrow

Hello,

I am finishing up a paper and I need to use the Patient's record as a reference in APA format. Can someone help me, I am not sure how to set it up.

Did they instruct you to do that? I've never heard of that. I would think that it would violate HIPAA to even reference a patients chart.

We just use the patient's initials. I am just not sure what else follows after the patient's initials.

Did they TELL you to include the patient's chart information as a source to be sited?

Kate. . .I just looked on the Landmarks Citation Machine site which I use all the time to help format my bibliographies. I think you just have to be creative. I would chose the "Anthology or compiled work" link there after choosing "APA" style at the left side of the home page and just fill in the applicable information they ask for their program to format a bibliography listing for you. I would only say that it is a patient chart, maybe using the name Patient Jane/John Doe. I'm not sure if you can use the medical record number or not. In any case, I've found that you can leave some of the fields blank where this program is asking for information and the program will just adjust nicely even though information it asks for is missing. Give it a try and see how it works out and if you like the suggested citation it gives you. It's a free service.
