Writing traveling contracts?

Does anyone have experience in writing contracts for travel nursing? Or do you know where I can get information on how to write a contract?

I would like to start working for myself. I have been in travel nursing for 1 1/2 years now and I seem to have to do most of the work anyway so why not do it for myself.

My questions are:

How do I find out how to write my own contract for travel nursing?

How do I know what to put into the contract?

How do I know what rates I should quote for the area of the states I am quoting a price for?

I am really excited to be my own boss but writing the contracts for the hospitals has me very nervous.
