critical care outreach

I scanned a few articles lately that have lead me to pose this inquery:

Is there anyone out there who has had experience in a facility where the critical care nurses have outreach teams that routinely round on the floors they regularly transfer patients to, with the purpose being to act as a resource to the floor/tele/intermediate care nurses?

If so, can you describe how this is implemented, how often do they come by, what you like or dislike about it from your perspective(define your perspective please).

I'd love to hear more from those who have this first hand experience!
