Hi everybody!

I am a nursing student and I am writing my concept map for my patient for Fundamentals. I'm stuck of what to write for my pt in Psychosocial. Pt is 80y female, orientated x0 (dementia), bed bound, contracted, and ADL care w/ 24 hr supervision. It is hard to tell if the pt might be depressed, distressed, or lonely since she does not talk. Daughter comes to visit as much as possible (at least once/week). What do you usually write in the psychosocial part for patients like mine? I searched online and looked at my NDH, but cannot seem to find anything. Maybe I am just looking in the wrong spot?

Does your care plan have to include psychosocial nursing specific dx if they aren't really relevant? Is this a hospital patient? Nursing home? Home health? It helps to have perspective on the setting.

I'd encourage you to examine your NANDA dx criteria and see if you can find an applicable dx. Or .. examine your patient. Write a list of what you perceive to be her psychosocial needs, then take it from there. There is also a General Nursing Student Assistance forum where you may have better luck.

My professor wants us to include everything on the patient even psychosocial, which in this case is a bit difficult due to my patient not being able to communicate and she does have a strong family bond. She opens her eyes and moans when she is in pain, but that is all. I will look in the General Nursing Student forum to see if I can find something. Thank you!
