Humana Discharge Planning Case Manager

Anyone worked for Humana as a discharge planning case manager? I have an interview in a week and a half, but I am not sure exactly what this position involves. All of the info I can find on allnurses is regarding discharge planners that work in hospitals. The recruiter that did my phone interview basically told me that the job is working with hospitalized Medicare pts planning for discharge. I am pretty sure the goal is to make sure health needs are met at home to prevent re-hospitalization. Any insight would be appreciated. Thanks!

I think a helpful thing to know for your interview is knowing what community resources are available in your state and/or muncipality so you can make referrals.("Office on Aging, Community Care Programs for the Elderly and Disabled, Meals On Wheels", ect') Also, know the Medicare guidelines for homecare eligibility, OASIS paperwork, etc. And, know things about home safety, DME negotiations, depression in the elderly. I think these are all big topics in home care.

Thank you for your reply, JerseyLilly! I have been a hospice nurse for almost 2 years, so I do have some idea of community resources, however I refer to my social workers for a lot of help in that area. I am just not sure what to expect working for the insurance company and all of the work being done in an office over the phone.

I am excited about the opportunity. I will pray about it, and if it is meant to work out, God will open that door. Thanks again!

I worked for Humana supervising case managers and I loved it. They are the best at making sure you have all the training you need and providing support. One clue, though. They will interview you by the STAR method, meaning that you will be asked about a "Situation or Task" from your work experience. They will expect you to relate the "ACTION" you took, either positive or negative, and the RESULT of that action. Your interviewer will need actual examples from your past. For instance, "Tell me about a situation where you had difficulty communicating with a co-worker" (you bring up whatever) then they say, "And what action did you take?" (you tell them), "And what was the result?" They don't care what you say you would do in a made up situation. You have to give real life examples, and what happened as the RESULT of your ACTION. Until the interviewer has the RESULT of your ACTION, the interviewer will ask you again a different way. The interviewer is trying to collect five stars from your interview. If you understand that you will probably be the one they want to hire.

susanthomas1954- I interviewed with Humana a week ago and felt good about the interview. I was able to provide a personal experience for all the questions they asked. How long does the process take until I hear something? Any idea? Should I follow up with the Humana recruiter in say 10 days... ? Any suggestions? I see that this original post is almost 8 years old but there's not a lot of recent posts about Humana. Thanks!
