So hard for new nurse
What is an IEN? I'm guessing DOC is director if clinical but in my 40+ years never heard it called that. I was hollered at by a doctor during my 1st shift in the hospital as a new grad. I knew I was right so I politely explained it to him again. The supervisor was standing behind him and verified I had told him the patient's diagnosis on the phone. He never apologized but he never rose his voice at me again. what good would a discrimination suit do? If I were you, I'd speak to the doctor about he thinks you specifically did that incensed him.

Can RN works as a floor nurse in Nursing home/LTC?
Of course RNs can work the floor in a nursing home. The last 6 places I ran (I travel) preferred to have RNs since it boosts the 5 Star rating.

New DON help with survey
You shouldn’t have lost your job over this. DNSs are leaving the industry in droves and being treated in this way certainly isn’t going to help

Help With New DON Position
From what I’ve heard, the focus for survey this year is everything infection control and immunizations. Make yourself a schedule knowing it will change depending on what goes on. Monday weights and skins, Tuesday Falls, Wednesday psychotropics review. Whatever will keep your mind focused. You’re lucky to have a supportive administrator. Feel free to PM me if you have questions. I’ve been in the business forever- tiny buildings, huge buildings. At the end of the day you have to go home knowing your residents got the best care possible

I got a verbal warning for not greeting the new Director of Care?
My first job as a nurse manager of a subacute unit--second day on the job. I had 4 admissions, 3 discharges, 3 people go out rescue-yes I was doing compressions (they all lived by the way) and at the end of the day, instead of a GOOD JOB I got a "Mrs. Jones was 10 minutes late to the hair dresser". I looked at the DNS and said, "Oh, sure. Now that I know your priority is hair and not living, I'll plan my time more efficiently." I was the DNS at a large SNF. A bunch of the managers and corporate people went bowling. I didn't go. I didn't go to another event because it was a blizzard where I lived...I DID go into work to finish the POC for the survey. The regional nurse came by. I got called into the administrator's office so she could talk to me. She told me I needed to be more of a team player and go bowling. I put my hand up and said, "Let me get this straight. I was the ONLY DNS in the region (14 other facilities near by) that didn't have to close my building because we had NO flu this Winter. AND, I was the only DNS who didn't get cited for side rails, and you're sitting here lecturing me because I didn't want to go bowling???" Two days later I turned in my resignation. She was SO shocked which proved even more to me how clueless she really was.

Patient complaint - Did I mishandle this situation?
I’ve been a nurse for almost 40 years. Unless the patient specifically asks to be woken up for medication, I wouldn’t wake them up for pain meds. I’ve also been a patient more times than I’d like. I have wicked insomnia. I always ask the nurses to leave me alone if I’m sleeping. They don’t always listen but some of them respect my wishes. From what you’ve told us, you did nothing wrong and certainly didn’t fail your patient

Long term care medical director is a jerk!
I've run into a few docs like that-and NPs too. I just tell them I'm following policy, protecting my license, and suggest they protect theirs. Then I ask them to repeat what they said so I can accurately get the quote for my nurse's note.

Best way around this: get an MD order and care plan it. Sometimes you get one shot to give someone with dementia their meds.

Understaffed and overwhelmed
The work load in LTC, especially for the CNAs won't change until something drastic happens. My facility has good staffing and luckily we've remained Covid free, but I've been in other facilities where the staffing was 2 aides for 40 residents on a dementia unit. TWO! Write to your governor, your congressman, who ever you can think of . Sooner or laterk they are going to have to listen.
