New to allnurses and MDS/looking for tools! omras, look aheads, etc

Hi everyone. I am new to the forum and MDS. I have been at this for 4 weeks and have a DON training me (some) but most of it is on my own...whew...it is not easy but I think I am going to really like this! Anyway, I would love any forms for medicare meetings....ways to track pps residents and then ways to track those going off pps with the "look ahead" period and "look back" periods...i am having a tough time putting this all together. I have experience in medicare so its not foreign but I was hoping someone might have a good way to track these patients who are "going off rehab" and maybe continuing on with Med A.

The snf I work for likes to see a column with "MD visits and MD orders" where do I find the regs on how many you need? this is what I have learned:

1 MD visit and 4 orders equals skilled care (given there is an actual skill...I know that part!)...and

2 MD visits and 2 orders???? is this correct?

AND>>>>>>>>>>>>>> a form to track it all would be great! Anyone have anything they could share? I would appreciate it very much!
