Question RE: potential case management position

I am a relatively new grad currently working on a surgical floor in a hospital. This is my first nursing job and after about three months I'm still not feeling this job. I have a long commute (just under an hour) and am realizing working 3 nights a week is not ideal for me. I thought I would put in a year or so there for the clinical experience and then look elsewhere but had a change of heart recently. I figured I would apply for the kind of job I'd ideally want and the worst that could happen is no one would hire me.

I got a call today to have a phone interview for a position as a RN Case Manager. The position would be working with clients in WI's Family Care program which serves adults 60+ with LTC needs and those 18-59 with physical/developmental disabilities. It sounds like there would be a fair amount of home visits, pt education, and care coordination.

I generally interview pretty well, but I'm wondering what kinds of things I should be prepared for in a phone interview and hopefully the in-person interview.

I have no experience with case management, only 3 months in clinical care (post graduation). I did work as a home health aide for a few years, which, while not directly related, helps me a little I would think. Any advice on what to prep for and what to ask about in the interview would be greatly appreciated.

Also, if this has been answered somewhere else, please let me know...
