New to dialysis
Ryan RN. If you think anyone can do it take a back seat. If everyone could do it then the turn over rate would be much lower, but it's not. Be honest. It's hard work and takes dedication. Im a tech working on my RN and started with 4 new hires myself, another tech, and two RNs. Only the two techs are left. Another nurse quit after one day back. Had another quit and go to DaVita for hire up position. It's not essy, it's real hard but rewarding.

Is this what being an RN in dialysis is like?
Sounds about right!! Except we have a 3 person assignment. My clinic is chaotic right now though like that too. I just hit 6 months and am not comfortable but not getting so nervous anymore!

Ideas to help my pt
If your continuously turning the BFR down to 450 then the MD orders will need to be changed. What's the point of using 14g needles if the BFR is continuously being turned down. Possibly NA profiling or adjusting temp to 36 with MD orders obviously. Are they diabetic and their sugars are low which is causing them to keep getting hot?

The interview was pretty basic my CM at the time asked questions asking if I knew what dialysis is, if I was good with computers and technology. She asked why i think i would be a good fit. If you are unsure of exactly what techs do look at the job description. Also check out the company's website before hand whether it be DaVita or Fresenius. I Youtubed videos about dialysis Txs and learned quite a bit!!!! Good luck

I Work for Fresenius and they Paid me for my training. I had no prior jobs in the medical field besides phlebotomy license, CNA certificate, and EKG certificate.

Nurses regularly assigned tech station
This happens in my unit more than often than not. It's unsafe and hard work for the nurses (I'm a tech). Today when I go in we have 15 patients and 5 people. 3 nurses 2 techs..... That should allot everything getting done with 3 nurses but usually it's 4 techs and 1 nurse.

Scheduling problems.
I'm a tech with Fresenius and I was hired for 30 hours a week even though its usually 40. My husband works 40+ as well. My hubby is now being pushed at work to work Saturdays and occasional Sundays. This means I will no longer be able to work Saturdays at my clinic. How should I go about talking to my CM about this. I feel bad she just took over our hectic clinic which is loosing employees for another clinic opening and some quitting. I've been doing my best to pick up extra shifts for her and what not. But I won't have the proper childcare on the weekends

important things to know about/ monitor for in dialysis pt?
I work as a dialysis tech. The thing we look for is recording vitals every half hour, during the half hour checks we review the machine ensuring all "chambers" are at the appropriate level and enough normal saline in the bag ( I'm assuming if your in the hospital the acute dialysis RN is doing this). We also do access checks, this is ensuring the fistula or graft has no dislodged needles or that any access is bleeding. For blood pressure it is different for each, most pt we won't let leave until there systolic is 100+. Heart rates tend to increase towards the end of tx. We have some pt who we can let ride in the 70s, others we can't. You need to watch their access hand, make sure there are no BPs being taken on that arm, no blood draws, pick lines, unless the DR has Ok. If they have a catheter then you need to find out if there is an arm being saved for a fistula or graft because if so we encourage no BPs to be taken on that arm. Signs and symptoms I would look for durning tax is yawning to indicate possible low BP. After fatigue in some PTs is fairly common, others feel great. They should not be lightheaded or dizzy, if so chances are they need a bolus of normal saline. I hope this helps. While I am a tech this is what WE monitor in chronic setting

Dialysis Staff Schedules
I work in a chronic unit with 3 shifts mwf and 2 on TThS. Your hired for 30 or 40 in my unit. I do 3 10 hr shifts (as a tech) which usually turn into 4 and my days are usually more. 11-12 hours. Nurses are hired for the same.

Training with Davita questions(RN)..
If their training is anything like Fresenius you will not be able to work full time elsewhere. I trained M-F full days between class room and otj training, maybe DaVita is different.

RAI to Fresinius...just looking for info!
Fresenius chain of command in my clinic is Clinical manager , we don't have charge nurses rather team leaders, then techs. We really all work together. Nurses usually have a 3-4 pt assignment on top of their nursing duties (depending on the clinic). Nursing duties i will assume are the same. Fresenius is an okay company. In my area they pay better than DaVita

Dialysis isnt for everyone
I started working as a tech in May of this year, I'm still new to the field. I have my CNA license, EKG Certificate, and phlebotomy license and will be applying for RN school shortly. I really do like my job and most of the patients,but I'm tired of being so anxious before work, and doing the same thing day after day after day, it is very repetitious. And as PCTs in my facility you have a lot on your shoulders, you don't only have your 3-4 pt assignment but usually try to help the RNs with their assignments too, but now you have the do the water room and more. I love the dialysis field but need more of a change daily, which is why I want to do acutes as an RN. I leave 5Am and my mother has to come watch my son and bring him to school for me, it's a lot of hassle because the only shifts they give are 6-5. My point is while I like dialysis I'm finding it near impossible to live my life. My husband works the same hours but he makes more than double what I do so it's not possible for him to do something else. If I want another child I'll never be able to work those hours any more . I feel like I've put so much time and energy into this job but I'm already tired of doing the same thing every single shift. What do you think?

I just had one interview not a phone, however I was hired as a PCT not RN. From talking with other RNs who were hired with me they had one interview with the clinic manager and the charge nurse was also present. I heard back a week later. I had an advantage because my CM had hired the secretary for a different clinic and she put in a good word for me.

I do like the company they offer tuition reimbursement and other reasonable benefits. There are several clinics in my area and the educational classes are far and few, I had to travel an hour and a half to get to it. However they reimburse you for gas mileage ( at least they did for me) and it was extremely reasonable. I find the pay average but the staff I work with is great... I'm hoping to transfer to the new clinic opening up in my hometown! If you need more time for training they will allow for that too. I was hired for 30hours a week/ 3 10 hour days. But during the training it was 40'hours a week. They mixed up the the training , the first week was classroom for 5 days, then week 2 was 4 classroom days and 1 on the job and so forth. Good luck it's a growing company who wants their employees to gain education.

Toileting during Dialysis
It is not possible. As a tech or nurse you have a 3 patient assignment, with my clinic there vitals are taken every half hour and before and after tx. We dont let patients leave with a BP less than 100. There is no physical way we can change a patient. Like one of ours who doesn't move, she just lays there, our chairs that they sit in do not fit in the bathroom and we our facility does not have the necessary supplies to toilet the patiets, not to mention there is no time in the day!

Hypotension
Every clinic is different, we have a pt that drops everytime but she carries ness with her and takes them to increase her BP. Other PTA we bolus with n/s, or give them soup which usually always works. Samething with Hypertensive PTs we have a couple that leave 190/90, they take their bp mess after dialysis, if they are feeling fine and the charge nurse is aware they will contact the md for further questions

I work for Fresenius also. The hiring process is very involved, as it should be. They pay you for 9 weeks of training, classroom and on the floor. Most likely you will train with a Tech first, then after doing that for awhile they would ease you into the RN aspects of the job. The training isn't easy but it can be done and it is worthwhile.

Make sure you know the basics of what dialysis is (obviously!). You will probably be asked how you are with computers/technology. It is a fast paced job, with long hours on your feet (most rn jobs are :)) it is a very repetitious job. What company is it with?

DaVita Interview
Congrats. It's a long 9 weeks of training, you won't be comfortable for at least year ( im on month 4) and I still get stomach aches on my way to work.
