Hello, I have been a nurse for 22 years and am now an AP RN with a specialty in Pulmonary ,Education and Critical Care. I have a keen interest in relocating to a very small underserved area which has been my dream since I graduated 22 years ago. My children are gone, and relocation is highly possible. What areas are good and would be recommended? Ive been scouted by Tribal Health care and many areas in the midwest. I also want to be in an area where there is some remote level of respect for nurses. i am interesting in doing some writing and publishing for journals as well. I have been active in local and international mission and really feel I belong there. Any advice would help.

It is terrible that hospitals are no longer a haven. The state of the economy affects psychological and mental states of all people. It is important to look at the circumstances surrounding the crimes to understand. I see this worsening with the economy we are in. I have seen violence in all units , not just the Er and psych. OB and peds unfortunately have their share as well. I know some hospitals with security outside of ICU.

Isnt it in vegas?

I have heard from many of my students who live there that Northern Va. is very competitive. The market is flooded with new grads. However, AHA has online classes for some things that are pretty good. I like their EKG class. I wouldnt recommend an online ACLS the first time around. YOu would still need to do your megacode and show skills (from an acls instructor) go to onlineaha.org. I have found it quite refreshing. I have had trouble with the Trauma classes and have been going out of state for those.

Hello, The NLN is pushing for instructors to be specialized in the area they teach both academically and clinically. That is why I chose CNS.

They are the top pulmonary hospital. I would give my right arm to get in as a CNS there.

I have not worked in IHS but hope to go to either chinle ( my first pick) or Crow soon. I have heard great things about crow. I have experience working in a military facility and we had too many supplies. Many of the doctors were old schooled but it was a teaching hospital and I saw alot. We actually were able to save a woman from a ruptured aneurysm with minimal cognitive deficits so that says alot. I appreciate any advice you can give me.

When I grad Nursing school in the 90s. I had 15 patients.

One other suggestion I have is to : join a club. Join AACN, ANA, ENA or whatever sparks your interest. This is something many people are looking for. What can you do to improve nursing?

Hello new grads and students: I am so happy to see so many wanting to volunteer. One real eye opening way to get some experience is to go on a mission trip. It adds a great deal to your resume. I have even been called in to interviews just because they are curious. You can work as an aide , CNA or care partner.

Hi, I have been a nurse for almost 20 years and I have seen both inclusion and exclusion. Unfortunately, the larger the unit, the larger the hospital, the worse it is most of the time. I think nurses are so eager to prove their worthiness that they can feel threatened by "new meat" I have been an instructor and had some units that are wonderful, appreciative, caring and loving with my students but others that were down right vicious. We need to help the new grads. Nursing school is so much harder now. They have to know so much more than us older nurses did. The patients are sicker, the acuity higher and the culture is much more tech. advanced. Please be kind.

How different is the CCRN from the CEN and the PCCN? Does anyone know how different the CCNS is from the CCRN?

You can get CO with non-invasive techniques . The NICOM pump does not require and a-line even. The Picco requires an A-line and a CVP

It would help to know how long youve been a nurses. Maybe slowing the pace down would help to start off . Maybe you could work less hours so you can absorb it better and give you time to think about it. Home care is a good option as usually you can work as little or as much as you want. Private duty another option.

Also, I am currently studying for cert. exams ( PCCN, CEN, etc.) and I have a CD I listen to in the car. It goes over details, questions, and I listen to it over and over and over.

Answer all the questions you can. Remember taking tests is a skill. It sound like you may have trouble with test taking skills or critical thinking. Hesi has a good review book. Lippincott has a HUGE question and answer book...nothing but questions. answer them all in the areas you are weak. You probably should know those...I can still tell you mine and I have been a nurse for 19 years.

Theres always private school. Anything has to be better than all that gas. Sentara pays well. I worked for them for years and am currently doing my advanced practice practicum there. I love Norfolk General. In Norfolk, West ghent and Granby are the best districts. Virginia Beach has many good ones but rents are high there. Also, out towards Suffolk on the Churchland/chesapeake line has some decent rent and decent schools. HOpe this helps.

The internships vary from facility to facility. Some are paid and some are not. Some you actually have to pay for but you get college credits. It is a grand idea for new grads. According to Benner( 2009), a focused education in one population does more for education than a million rotations. I think thats why many students are so " lost" when they graduate.

My sister lives about 70 miles or so from DC. Her husband works in the district and the closer you get to Dc the more crime is. Thats why they live in the boonies.

Try home care

I live in Virginia but am moving to AZ for a number of reasons one being a job in the field I want to work in. Anyway, from a management perspective, I would like to offer some wisdom.1. The cost of orienting a new RN is about 75,000. This is a strain on many facilities especially those that are struggling right now. 2.There is a huge gap between education and practice. There needs to be a revamping of the nursing education system so students can transition better. 3. The board pass rates are dropping overall in the nation. 4. Many nurses are not prepared and unable to practice safe care with limited resources and staffing. 5. Litigation is increasing in many areas. I have seen many lose their jobs after their 90 days because they cant function in their new job. I, personally, try to work with them as long as it is feasible.

I recently took a compassion fatigue instructor course as part of my trauma/disaster training and it was found by Figley and Figley as well as some studies by John Hopkins that a loss of sense of humor is the first thing to go.
