A transcript that is acceptable to both the nurses boards is one that is filled in at the institution you did your training and sent directly to the board. Most of the transcripts show how many hours of different specialties you did in the practicals what theory you did and the grades you got.You cant just write it up and send it.Ihope this helps

Moved to a thread on it's own as nothing to do with the original thread of transcript for Australia
The transcript has to come from where you trained it will not be accepted by the NMC from another body. If you can't get hold of anything because your school no longer exists do you have a governing body that you have to register with?

people help me! (if i'm in a wrong theme, tell me where i can find it) here is a situation. i’m a rn. i have my 9 years in the critical care unit. i’m a certified nurse-anesthetist. i received my medical degree out of the us. now i’m working on my us nursing license. this is a long process and not easy to prepare and to send all my paper from my country to the nursing board. anyway, all my documents have been sent to the nursing board of ca for the certification and permission to take nclex. after 9 months, i’ve got a letter from the board that i do not have enough hours in psychiatry. i took psychiatric nursing in local university, here in ca. but, comparing to the university’s curriculum, in my diploma, in psychiatric nursing, i have more hrs than they are.
i still do not know how many hrs do they need exactly? the ca board even didn’t evaluate my diploma (according to their internet site they do so). instead they sent me a letter that i do not have enough hrs and my work experience doesn’t count!!!
so, i have to evaluate my diploma through cgfns, which i’m doing now. another dilemma: the process is very long and slow. i’m afraid to lose my qualification. i cannot work without license, but my education and work experience should give me some possibilities to work. but instead i have to be on the student visa and support my status.
if anybody in the same situation or passed the same process i will appreciate any suggestions and help here or trough pmessage! how did you do your transcripts for evaluation? did you separate subjects like anatomy-physiology, or gynecology-obstetrics and how many hrs in each? – in my diploma i have them as the one subject.
…a lot of details in the paper work that i might do not understand. sorry, couldn't make it shorter. but, please help! thanks!

hi! could it be that the transcript of training = transcript of academic record? this should be from the institution/school/college/university where you got your qualification, and according to Silverdragon102, sent directly from there, to the requesting body, and in your case, the NMC.
ideally, this document must include:
a. your full name
b. the start and end date of the course,
c. name of course and details of course content = subjects taken, # of units, may be from the syllabus
d. number of hours of theoretical instruction and clinical practice = lecture, related learning experience (RLE)
e. signature and official stamp from the relevant course provider.
this is similar to the information requested in a CGFNS/VSC form, if you are familiar. your education provider might be familiar with this, and may require a fee upon your request (at least, from where i am from).
to anyone who can add to the same, please feel free :typing

many states will not recognise work experience if you are short on some hours with your transcripts. if the state is asking you to make clinical and theory hours up you could contact your local nursing school and see if you can attend as a guest student.
if you are on a student visa how are you planning on staying and working? what sort of visa are you planning on applying for?

Why not apply to be an MD in the USA? Wouldn't that make the most sense? You need to have nursing education not medical education to be a nurse.
Also note the California Board of Nursing is working reduced hours. Plus what state is your original US nursing license.

Obviously he/she does not have an original US nursing license, which is why they are applying for one....? Dophamin, do you have a SSN? Where are you a certified nurse-anesthetist?
… I have my nursing license and I have nurse-anesthetist certificate – all I got in Russia. So, now I’m on student visa during my paper work. No SSN yet. …. It took me already almost 2 years and I just tired of all process… I’m a new on “allnurses” and if anyone passed same process, please, share it with me or PM me. I appreciate any help.
By the way “transcript of training” – looks like table: Name of discipline/Theory clock hrs/Practice hrs/Grades
I just have to clarify you went to nursing school and then medical school ? What school are you attending now?
i think i didn't say it correctly. my education: nursing college, nurse-anesthetist school (my experience is 9 years in ccu - reanimatology-anesthesiology unit - of major hospital in the town with 1.5 mln population) and master degree in "health services management". i'm on student visa here and have to support my status by taking classes, but at same time i'm working on my license collecting documents for us boards...
why ca board is working reduced hrs?
