So, do you care plan the target behaviors associated with antipsychotics? I am in the process of reviewing our care plans to include non medication interventions. Now if the census will stay up so I can have enough staff to implement them.

I work for a CCRC so I don't receive any outside admissions. Therefore, I have a pretty sweet set up. And our LTC unit maxes out at 24. Its also a private pay facility so charting is at a minimum.

Novant pays slightly more than Carolina Healthcare. CHS starts out at 19.65 per hour. I think 20.19 for Novant. This is the last rates I heard of for new grads. However I work in LTC so the pay is a little more than the hospital.

Brandon is right that you can't change the ordered dose if it is scheduled and not sliding scale. It's either all or nothing.

Pay Rates??
I started in LTC after I graduated in May 2011. I made $22.50 an hour plus a shift diff of $1.00 per hour for 7p - 7a. I switched facilities after 6 months and started there at $24.00 per hour. I have now been there for almost a year and am about to receive my first raise. Also, I live in NC as well.

I have 18 months experience and I currently make $24.00 per hour on the 7a-7p shift at a LTC facility as a RN.

I am in NC and I make $24.00 per hour in ltc working 7a-7p. I graduated 15 months ago from nursing school. The local hospital starts new grads off at $20.21 per hour plust diffs.

How long was you orientation traing when you started in LTC?
I received 2 days of shadowing and then I was on my own. At my current job, the nurse who was going to train me called in on my first day, the DON had careplan, so I passed meds based on pics in the MAR and by asking CNAs.

Will I ever get out of LTC or am I doomed?
It's not just you. I feel doomed too, like I forgot every skill that I learned in nursing school. I do take care of my residents. I just sometimes wish that I could have worked in the hospital. When I have lunch with some of my former peers and hear all of the exciting things they do, I feel a bit jealous. The only thing I can contribute that is exciting is doing CPR on a 90 something year old resident whose family could never agree to sign a DNR. Until then, I am going to make the best of it and give it 100%.

Barrier Cream on a Skin Tear?
I can't even begin to imagine a CNA trimming off skin that wouldn't stretch back. I clean skin tears with normal saline. Apply steri-strips. Cover with Telfa and secure with Kerlix and monitor daily for s/s of infection. I have seen some nurses apply steri-strips, cover with tegaderm and then change q 3 days instead of just monitoring. When they change the tegaderm, it just pulls off the steri-strips and reopens the wound. But I am definitely fine with tegaderm for monitoring.

I took most of my prereqs including A&P I & II before entering. I did however have to take Micro, Nutrition, and a humanities class. However, there are some girls in my class that took everything from PSY, micro,and A&P while taking nursing classes. I think they were pretty bogged down though. Micro is not bad at Cabarrus College. I had no problem getting an A while taking nursing classes. If you don't have children yet and don't work fulltime, then you should have no problem taking some additional classes with your nursing classes.

I was definitely thinking assess before implement. Sounds like the instructor forgot the nursing process. That one should have been thrown out.
