Regarding the anemia you could use altered tissue perfusion r/t low RBC,H&H. although I don't know her vitals. There is evidence of this with the fatique's easily

Those are reasonable diagnosis. You can use infection since it says possible infection in the medical diagnosis. She is also at risk for further infection because she is a diabetic since sugar feeds bacteria. She is also at risk for injury r/t her med use (BP meds and oral diabetic drugs) and from her surgery(mobility issues). These are just a few we would use on the unit I work at.

May I ask if you know why they have her on 3 blood thinners. Are they trying to get her to therapuetic coumadin level. I work in geriatrics and seeing how the elderly bruise easily, that just sent up a red flag for me.

Those are all good dx, but I would think along the lines of ABC. Your first priority post-op is to prevent respiratory problems. I would add one r/t to that.

Frustrated with clinical experience
Wow, I don't know what to say. In my program we have been doing the skills from the first semester. I am also in my last semester, and every clinical instructor that I have had has made us responsible for total patient care. This means we did everything for that patient (meds,foleys IV). The instructor I have now told us last week that we were to do everything except hang blood with the patients primary nurse. Meaning she did not have to be in the room with us. So basically we have started our preceptorship.

ATI Maternity
Hmm, so you want someone else to do the work for you. I am not trying to be smart but come on. We had other test to take also. I had two finals and two ATI proctored test to study for. If you have payed attention in class and have done the practice tests you will be fine. I made a level 3 on mine and did not really study the ATI book.

No she did not just ask the instructor if the NuvaRing itself went off after the instructor explained that they used to come with a timer that went off when it was time to change them.:eek: I wish you could have seen the look on the instructors face.

What I want for christmas this year
I am going into my last semester, hopefully I will be graduating in May. I have explained to everyone that the expenses for graduation will be great and if they could just give me the money they would have spent on the gifts it will be greatly appreciated. If they wanted to buy something instead, please buy my pinning dress and hat, or a Kaplan review course.

Missed Clinical Orient. May be Expelled
My school is good at keeping the student informed about schedules and changes. However, the instructors will tell you that if there is any confusion about schedules it is our responsibility to clarify it. I do not understand, if she knew that orientation was coming up, why she did not call or e-mail the instructor to get a date and time. And, if everybody else was there why wasn't she.

Should I argue this? about IV push test
I looked it up in my drug book and according to mine your instructors are right. Here is what mine says. IV Push: To prepare famotidine intravenous solutions, aseptically dilute 2mL of famotidine injection (solution containing 10mg/mL) with 0.9% Sodium Chloride Injection or other compatible intravenous solution to a total volume of either 5 mL or 10 mL, and inject over a period of not less than 2 minutes. If you notice it says to a total volume of either 5mL or 10mL. The only way I would fight it is if you can show that yours says something different

OK then look at the UTI diagnosis. How was her output? Was she incontinent? If she had not voided since she had left the other hospital she may have Impaired Urinary Elimination. A piece of advise never assume anything when it comes to health care. Even if she had not had any oral intake, she would still have fluids from the IV in her system. I think there needs to be proof that she has impaired physical mobility other than the two falls. There could have been many factors involved in her falls. Did you see her ambulate? Did she really have hip surgery? If she did then what about Impaired Skin Integrity or Risk for Infection. I am not trying to be hard on you, just trying to get you to see the bigger picture.

I'm sorry but I disagree about her ABC's being good. She has HTN, Hypokalemia, and pancytopenia (which is a reduction in all formed elements of blood). All of these can be a factor in her falls. I would go back and look at the ABC's again.

I am 35 years old and am in my second year of my program. I have two more semesters to go. I too thought I was to immature to know what I wanted to be when I grow up. My grandfather helped with that decision on his death bed. He said to me " It is always easier to listen to the negative thoughts in your head than to have the courage to pursue your dreams. Remember if it is to be it is up to me." It took me a while to figure out what he was talking about, but I finally realized that he knew before me what I wanted to be when I grew up. Every time I want to give up I can hear him saying those words and find the courage to go on. Just also remember you can do this.

Worried about instructors
This is the norm for my school. My school is part of a consortium with two other community colleges. We arrived at classes Wednesday not knowing who our instructors were or even where our classroom was (I am a 2nd year student). But from what I have seen the two instructors we have are the best so far. Remember the saying "Don't judge a book by it's cover." You will find that most instructors are there to help you succeed and will do what ever it takes to make sure it happens. We also had to attend new student orientation so that we could mentor the in coming class. The only instructors there were the directors and their assistances of the program at the three colleges.

My ATI kit came and I'm a bit lost
My school currently uses ATI. The CD is to show you the skills and the books are supplemental to the text book. We have to read the ATI chapters that match what we are studying and then take a non- proctored online test before we can take the in class exam.

Help Understanding Test Question
Our instructors are somewhat slack when it comes to care planning. In our fundamentals class we had a short class period where someone explained documentation and concept mapping. Other than that we were told to buy a concept map and care plan book and read those. Our instructors do use goals and outcomes interchangeably. I think that is why so many of us are having a hard time with these types of questions. I forgot to add, my clinical instructor made us do concept maps on our patients every week. But we did not get to do an assessment on them before hand. So we were basically doing them from the medical diagnosis and abnormal labs that may be two or three days old. But at least this is better than the other clinical groups who did not have to do any except for the one required for the case study.

I am in Mental Health this semester. We are currently studying eating disorders. I am having trouble with this question. A client's disturbance of body image is evidenced by her claim's of feeling "fat" even though she is emaciated. The outcome criterion for this target behavior would be: A) Consuming enough calories to sustain normal body weight. B)Ceasing a strenuous exercise program C) Perceiving standard body weight and shape as normal D) Demonstrating ab absence of preoccupation with food My study group is torn between A and C. Any help you can give will be greatly appreciated.

Parents at Orientation
My college had two phases of orientation. The first was for the students only. The second was for family members or whoever we wanted to bring. We did not have to stay for the second part if we did not want to, but thank god I stayed and my husband showed up. The instructors did all the explaining of how hard my life was going to be the next two years and how he was really going to have to help around the house. This was also were they told the students they were going to have to let some things go and let others help out. One instructor had a good time management presentation for the loved ones. They really got to see that we really would not have a life outside of nursing school.
