  2. Hi everyone! Im doing a careplan for a newborn neonate who was born to a mother positive for GBS and also had PROM. My nursing diagnosis is risk for infection, im trying to come up with interventions specific towards newborns, all i have is handwashing, clustering cares, noticing signs and symptoms....does anyone have any other suggestions for interventions?? any help is greatly appreciated! thanks!!
  3. So i am wondering if anyone can tell me how safe i is to take adderall and a sleeping pill, is it almost an overkill? Also i was wondering i had a pt. who has been on adderall for 4 years, and is just now begining to experience high blood pressure, anyone know some reasons for that?
  4. I think i would find out what med/surg book youd be using during the semester and order that to look at, that way you will have your book for classes, and can get a start early. The text books, i think, give a really good start for beginers!

I actually think its really possible, in my BSN program, we did have a physical assessment class before we started doing careplans, but honestly i havent used any information from my assessments other than vitals and for my AEB in the NDx to do my careplans, and they all turn out really well :)
  6. I am almost done with my J1 semester, and for some reason ive been thinking i dont know if nursing is really what i want to do now. I mean i love the thought of the medical profession, the human body, helping people, and forming those helping relationships. But it seems that so far what i have learned in clinicals is exaclty what i dont want to do. I dont want to base everything on meds, i dont wanna chart, and i feel that this is what every nurse in my rotations so far has done. I understand their are a bunch of fields to go in to...but im just having some doubts. It just seems like nursing isnt what i thought it was. Anyone else feeling doubts after they put in so much time effort, and of course money in to it? Any advice to kick me out of this mood??
  7. Thanks everyone!! You all helped so much! The OR was so cool, after the day was done, i think i had a ton of ndx that came to mind, ones that i just wasnt thinking about since i didnt know what all would happen before, and most of them were ones you all came up with, so thanks again!!!!
  8. I actually used risk for infection, and impaired skin integrity, and risk for impaired tissue perfusion for this patient already. My instructor is having us come up with 4 ndx, which is why im having so much trouble with finding another realistic one. Im just thinking as long as i have something half way realistic sounding it should work...right...??? Thanks for the help though!!!! It helps alot!
  9. Hello all nurses, students, and everyone! So i have my OR rotation tomorrow and i am having trouble coming up with a realistic goal for my NDX, i was wondering if anyone could help! I actually dont even know if my NDX is a decent one...so if there are better dx im all ears. So the surgery is a total shoulder arthroplasty, my dx is risk for bleeding. I feel like just saying "pt. will maintain minimal blood loss, or pt. will maintain stable vital signs" are just unrealistic in this situation... Any help is greatly appreciated!!!!
