Those are reasonable diagnosis. You can use infection since it says possible infection in the medical diagnosis. She is also at risk for further infection because she is a diabetic since sugar feeds bacteria. She is also at risk for injury r/t her med use (BP meds and oral diabetic drugs) and from her surgery(mobility issues). These are just a few we would use on the unit I work at.

May I ask if you know why they have her on 3 blood thinners. Are they trying to get her to therapuetic coumadin level. I work in geriatrics and seeing how the elderly bruise easily, that just sent up a red flag for me.

Those are all good dx, but I would think along the lines of ABC. Your first priority post-op is to prevent respiratory problems. I would add one r/t to that.

Frustrated with clinical experience
Wow, I don't know what to say. In my program we have been doing the skills from the first semester. I am also in my last semester, and every clinical instructor that I have had has made us responsible for total patient care. This means we did everything for that patient (meds,foleys IV). The instructor I have now told us last week that we were to do everything except hang blood with the patients primary nurse. Meaning she did not have to be in the room with us. So basically we have started our preceptorship.

ATI Maternity
Hmm, so you want someone else to do the work for you. I am not trying to be smart but come on. We had other test to take also. I had two finals and two ATI proctored test to study for. If you have payed attention in class and have done the practice tests you will be fine. I made a level 3 on mine and did not really study the ATI book.

No she did not just ask the instructor if the NuvaRing itself went off after the instructor explained that they used to come with a timer that went off when it was time to change them.:eek: I wish you could have seen the look on the instructors face.

What I want for christmas this year
I am going into my last semester, hopefully I will be graduating in May. I have explained to everyone that the expenses for graduation will be great and if they could just give me the money they would have spent on the gifts it will be greatly appreciated. If they wanted to buy something instead, please buy my pinning dress and hat, or a Kaplan review course.

Missed Clinical Orient. May be Expelled
My school is good at keeping the student informed about schedules and changes. However, the instructors will tell you that if there is any confusion about schedules it is our responsibility to clarify it. I do not understand, if she knew that orientation was coming up, why she did not call or e-mail the instructor to get a date and time. And, if everybody else was there why wasn't she.

Should I argue this? about IV push test
I looked it up in my drug book and according to mine your instructors are right. Here is what mine says. IV Push: To prepare famotidine intravenous solutions, aseptically dilute 2mL of famotidine injection (solution containing 10mg/mL) with 0.9% Sodium Chloride Injection or other compatible intravenous solution to a total volume of either 5 mL or 10 mL, and inject over a period of not less than 2 minutes. If you notice it says to a total volume of either 5mL or 10mL. The only way I would fight it is if you can show that yours says something different

OK then look at the UTI diagnosis. How was her output? Was she incontinent? If she had not voided since she had left the other hospital she may have Impaired Urinary Elimination. A piece of advise never assume anything when it comes to health care. Even if she had not had any oral intake, she would still have fluids from the IV in her system. I think there needs to be proof that she has impaired physical mobility other than the two falls. There could have been many factors involved in her falls. Did you see her ambulate? Did she really have hip surgery? If she did then what about Impaired Skin Integrity or Risk for Infection. I am not trying to be hard on you, just trying to get you to see the bigger picture.
