How long does it take you

average time to write up a careplan?

Featured Replies

4 hours. It takes me what seems like forever.

It takes me about 2 hours to do the prelabbing part of it (meds, labs, etc) and about 2 hours to write up the the other part (Gordon's, nursing dx, etc). So, 4 hours total.

Several of my classmates write out all 12 pages by hand, and that would take me FOREVER.

average time to write up a careplan?

I made a template on my computer & that saved me alot of time. I can go back & use my drug sheets, patho ect if I have the same ones again. It usually takes me 4/5 hrs. depending on how many meds I have.

Almost all the places that i have worked, the wards just used pre-printed careplans that were specific to the ward speciality, so all it takes is customising to the specific patient, which doesn't normally take more than 30minutes, if you include risk assessments for pressure sores etc.

1st semester they took about 5-6 hours. Now in my 3rd semester I'm down to 3.

instead of careplans, we do concept mapping...and it takes me on average 3 hrs

We do a concept map, care plan, and a pathophys paper on each patient assignment. Takes for-freaking-ever but I'm sure you're looking for something more specific, so I'd say 8 hours easy.

What is a concept map? When u guys are talking about care plans, are u referring to the diagnosis (_____ AEB_____RT) with your goal, nursing interventions with rationale, and evaluation?

We do what is called a patient profile and it includes the above as a part of it. We also have to have detailed information and explanations for the patients diagnosis, past history and treatment, labs (normal values along with whats abnormal and why), procedures with full explanation and result, drug cards on all their meds....and the list goes on! agggghhh. Ive done two so far and the first one took me around 12 hours....the second one took me around 8. I HATE PROFILES! They are soo time consuming.

The careplan itself I can do in an hour or two but the process of the profile, which I think is the same as what u call a concept map, its hours and hours. I end up with around 20 pages single spaced! this has been the thing Ive hated the most about nursing school, Ive gotten used to all the studying and tests, but the profiles are driving me crazzzzyyyy.

Our concept map consists of clustering the abnormals...then focusing on the 2 main clusters and writing a nursing diagnosis on that....like the R/T, AEB...etc. Then we have to write goals. Takes about an hour and a half. I don't think we are doing all of the concept map yet though. I beliee there is more to it we just haven't gotten there yet...i'm only in my first semester of PN school

Careplans probably take me from 1-2h to put together - that's after gathering all the needed data from charts, etc. :)

We have to do pt history, labs, drugs, patho & 3 nursing dx/careplans for each patient and we take 3 pts a week. I'm 3rd semester, so I can usually reuse careplans or pathos from previous semesters. That really cuts down on the time. It usually takes me about 6 hours to complete all the paperwork.
