Funny chart entry

Back in nursing school I was reading a chart of an elderly woman on the medical unit and came across an abbreviation I could not figure out. The doctor wrote that the patient was HAAL?! This was before we had GOOGLE so I looked it up in my text books but could not find it. I asked my instructor what it meant and she had no idea. The charge nurse could not explain it. My instructor directed me to call the doctor. He started laughing, explaining that the lady was HAPPY AS A LARK. LOL!:)

I read a lot of our doctors typed notes, especailly if I have to care plan a patient I dont know much about. He always puts little "easter eggs" in them, the more days he has been on call, the funnier some of the notes are.

I like some people's nursing notes that start off "received patient in bed..." Oh really??!

I seen one on Facebook or Pinterest. Chief complaint was "unable to eat diarrhea" lol shows how important punctuation is.

An MD dictated a Female Patient's injury suffered during the felling of a tree and its subsequent cleanup, stating, "The Patient was struck by a branch between the legs".

The Medical Transcriptionist could not understand the MD's thick accent and typed, "The Patient had a stroke on a bridge between two lakes".

Where I work we have lady who just can't spell (not a nurse) and she frequently writes, "patient poisoned in bed", "patient poisoned in w/c". She means positioned, at least I hope she does. Lol.

We have a resident at our NH who is extremely OCD and a hypochondriac. He has been working on a letter for the doctor addressing his supposed medical concerns for over a year.

In his physician's notes in the chart our doctor refers to this letter as the resident's "magnum opus".

Our septuagenarian physician routinely uses LOL as an acronym for "Little Old Lady" in his charting.

I'm pretty sure he's completly unaware that LOL has taken on a different meaning for the rest of the world.
