Funny chart entry

Back in nursing school I was reading a chart of an elderly woman on the medical unit and came across an abbreviation I could not figure out. The doctor wrote that the patient was HAAL?! This was before we had GOOGLE so I looked it up in my text books but could not find it. I asked my instructor what it meant and she had no idea. The charge nurse could not explain it. My instructor directed me to call the doctor. He started laughing, explaining that the lady was HAPPY AS A LARK. LOL!:)

We have a resident at our NH who is extremely OCD and a hypochondriac. He has been working on a letter for the doctor addressing his supposed medical concerns for over a year. In his physician's notes in the chart our doctor refers to this letter as the resident's "magnum opus".

Oh my gosh!!!! HILARIOUS!!!

On a paeds ward we've had a bank nurse put in the notes 'patient states no social worker - yeah right'.

The patients and parents can read notes at any time they want while they are in hospital, plus the notes are kept for 17 years in case of complaints, court cases etc...:up:
