School Nurse Forms

Can anyone help me with a simple log for health room visits. Any type of log sheet template will be great!!!!! I just want something simple with numbers and lines, I can edit as needed.

I have a health office visit form (one sheet) for every student visit. It was used in the past, but I don't use it since I document/track office visits in SNAP. Happy to send it if that is what you are looking for. (I can be poked via PM with your email address if you would like).

In my sub job where it was even busier than I am now, I had a sheet of notebook paper and would write down down: each student's name, grade, complaint, treatment. I just divided my paper into 4 columns (sometimes 5 with a time in/out column depending on whether or not I was subbing in the HS or elementary school). As a sub I didn't always have computer access, and even when I did and many kids came in, I did this so I could document it in the computer when I had a moment.

You can also set up an Excel spread sheet. Then you can sort by columns to pull specific information. Complaints, dismissals, student specific etc. Good Luck!
