RN's without APRN certifications working in capacity of NP

I would like to know if any of you are seeing this trend where physicians are hiring registered nurses to function in the capacity of an Advanced practice nurse. I know of several situations where a registered nurse is hired by a provider to round on patients and write orders as if they were advanced practice clinicians when in reality they are not.

Please respond with input or personal experience with this problem

I don't have any experience with this, but if I had evidence that a RN who did not have an advanced practice license was writing orders, I would protect the public safety and report it to the BON.

Are you sure they aren't working off a protocol that you just don't know about?

Nothing new here. RN's round all the time, using their surgeon's protocols to write orders.

My ortho surgeons (note: personal experience as requested) trusted me to use my experience and their protocols as needed while rounding.

As long as they're working under established protocols or writing orders as verbal or telephone orders from the physician, and not representing themselves as having credentials they don't have, what exactly are they doing wrong? (I'm not saying I think this is a good idea, just that I don't see what kind of argument could be made against it.)

LOL, you would be wise to understand exactly what you are reporting, so as not to make a fool of yourself. RN's rounding on post-op patients and writing scripts per protocol for post-op care is standard practice.

My first thought was "Standardized Procedures."

Well, care coordinator for my surgeon. Ensuring continuity of care with PT/OT/home care for our patients. And yes, using established protocols and nursing judgment to prescribe future care.

The nurses may not be imposters or doing something illegal. They use protocols and communicate with MD and get physician approval for orders. I work in dialysis, and we work from protocols or algorithms that contain every imaginable scenario. We do assessments and base care and medication administration based on blood test results and the physician's signature on the algorithm.
