Not sure if someone else was supposed to be posting a question... sorry if I'm invading someone's territory :) Though I'd throw one out there since I'm reviewing NCLEX questions anyway:

The nurse has been teaching a new mother how to feed her infant son who was born with a cleft lip and palate. Which action by the mother would indicate that the teaching has been successful?

A. Placing the neonate flat during feedings

B. Providing fluids with a small spoon

C. Placing the nipple in the cleft palate

D. Burping the neonate frequently

I'd say C. I think breast feeding is encouraged as long as it is possible.

My guess is D

D. With a cleft palate, the infant will be swallowing a lot of air. Burping will relieve some of the discomfort from the air. -Andrea

D. b/c of the neonate swallowing lots of air at feeding. I believe that they feed with special nipples as well that are longer and the hole is a little larger that doesn't require as much ability to suck as with a "regular" infant.

We were taught the ESSR method: enlarge the nipple, stimulate sucking by rubbing the nipple across the baby's lower lip, swallow, rest to allow the baby to finish swallowing the breast milk or formula.

Since none of those are provided, I'll go with D. burping frequently minimizes air swallowing and GI flatus.

Well you wouldn't want to place the infant flat or use a spoon. Placing the nipple in the cleft palate wouldn't be right either so the correct answer has to be D. They suck in extra air

See, after I saw the responses I had a "DUH" moment. That doesn't really count, coz I woulda gotten that one wrong. Man o man, I need some serious studying before I take the NCLEX!

ANSWER:

D. Rationale: Because a neonate with a cleft lip and palate can't grasp a nipple securely, he may swallow a large amount of air during feedings and, therefore, require frequent burping. A neonate with a cleft lip and palate should be fed in an upright position to reduce the risk of aspiration. Spoons aren't used. A neonate with a cleft lip and palate may use specially prepared nipples for feeding. Placing the nipple in the cleft palate increases the risk of aspiration.
