Just curious- When pt's are drug tested..
We do not usually report it. If an OB tested positive in the office then usually they have already been reported to a social worker by the doc. We test them again and if they test positive when they come in to deliver then usually the LD nurse or doc calls in and they do not get to leave with their baby. We are mandated reporters. I have often wondered why we do not report all these 20 something y/o men whose teenage girlfriend delivers. The psych patients we stabalize and usually within 24 hours we ship them to rehab or make them an appointment with community counciling. Now if they show out at the hospital and a policeman has to come keep watch I have seen a few end up infront of a judge I guess because of the police report.

Does reviewing previous courses help before going back to nursing school?
Well it never hurts to be prepared. I would definitely go over pharmacology.

holding lasix d/t BP reading
At my hospital it is based on parameters outlined in the chart or MAR. If there are no parameters then someone would have to call the doctor. Now if a different doc is making rounds it may be based on a different opinion. Sometimes there may be a change in diagnosis... suppose the pt had a stroke while admitted. You would think it was due to their bloodpressure so you want to treat the BP.... in actual fact in most cases we keep the pressure higher to increase bloodflow to the brain. Another thing that comes to mind is if the patient starts getting really tachy then the doc may think they are getting too dry and hold the dose. The best thing to do is ask questions of the RN about the held dose. Its the best way to learn.

Career Advice
Well it is only a 500.00 per semester tuition reimbursement. We are short staffed right now so unless I am in the pool I will not be able to get the kind of time I need to study. When you work nights that first day your off.... well you sleep it away. There are two other hospitals about an hour away that I could pick up pool time or work the two day weekend option (WO really sounds good and I could keep benefits... but the drive, ).

Career Advice
After being a diploma nurse for almost 8 years I have decided to get my BSN inorder to open up my options in nursing. It seems like all the openings I see require a BSN. Considering getting my masters. I have a few classes to take and then I can start a local online program in the summer. It will take 1 year. I am considering working poole. I can get on my husbands insurance. I just think it would be nice to work when I want too while taking classes. Has anyone had any experience with poole?? Pros and cons?? I know I will lose my benefits and will be compensated by higher pay. A friend of mine works at a larger hospital and they are about a 45 minute drive but they could use me for poole or a weekend option which is friday day and sunday night.

New nurse needs advice
I would have asked her to please help 314 with the commode and took care of the HTN patient myself. You are delegating so she is not qualified to help with bloodpressure meds but she can help someone with a bsc. Praise her when she does a great job and make her see she is a valuable person on the team. I know that taking vitals and bsc are a large part of their job description but unfortunately they see alot of nurses who are not willing to help in those areas. That being said if she is disrespectful or things get worse it may need to be addressed.

Please help!!!need a good advice regarding nursing school!!!
Nursing school is very stressful. They separate the goats from the sheep. The accelerated programs are fast and furious. The program I was in had medical math and if you made less than an A you retook the test and were kicked out if you didnt make an A on the second attempt. Please think long and hard before you go into this area of study because I promise you if you think school is stressful wait until you are taking care of people. Have you though about teaching school??

Need some encouragement!
So glad you were able to finally get some information. I feel for you. I have a friend that was the single mom of two. The oldest a teen who got mad at mom for taking away her phone. Right before graduation she was arrested for child abuse. Her daughter told her paternal grandma she was abused to get back at mom. Grandma called the school and the police. The school investigated as well as the state. the BON told her she could not even work as a cna until cleared. They had to move to low income housing, and the teen was removed from the home (had to go to a scary girls home, LOL). It took from September - july for her to get cleared (graduated in december) and to get her daughter back. She is happily married now and has been working as a cardiac nurse for several years. The home took the brat out of her daughter who missed out on cheer camp, and alot of other activitys. Thank God they are all doing fine now. Hope you begin work soon.

Help! Acls Class
Well I passed my acls class and boy was it hard. I was the only med-surg nurse and everyone else was icu, micu, nicu, etc. When I did my check off they based the senerio on someone coding on the floor. I still had to take the leadership role and tell the team what meds to push, etc. Anyone taking this for the first time should get their book in advance and study the meds, rhythm strips. and read the practice senerios. When you tell the team to push a med you have to give a dosage, tell how it comes, and the max dosages. melissa

What was the MOST ridiculous thing a patient came to the ER for?
We had a mom come in with her infant because the infant had a diaper rash. She called in a complaint because others were seen before her. a code, a fx femur, a broke hip, a decreased loc, and a chestpain. Somehow her babys diaper rash was more important even though there is a sign that says patients are seen based on need. melissa

Help! Acls Class
Thank you for the information. My class starts tomorrow at 0735. It is almost a 2 hour drive so I got a room at a hotel the hospital runs for a very cheap price for tomorrow night. I guess what worrys me is passing the test where you have to be in charge of a code. Of course there are those that always have to tell you the horror storys they have heard.
