NEW TO NEURO ICU
The neuro ICU I work in was ranked #12 this year in US News and World Report. The hospital is a level 1 trauma center, Magnet, and primary stroke center. We take care of very complex as well as rare neurological conditions. However (in my opinion), a well rounded advanced practice nurse should have more than one year of ICU experience, especially in an area that is not so specified to one patient population.

Floating ICU Nurses
My unit (neuro ICU) is the only unit required to float to a lower level of care in our hospital. We float to the neuro step-down floor, though they did not float to the ICU. Max is 3 patients and only 2 can have drains. A friend works SICU in a hospital in West MI and the ICU nurses are required to float to the progressive care floor. I worked this floor as a nurse tech during school and it's basically a MICU step-down minus the gtts,etc. All patients are tele and usually total care.

Mandated Flu vaccine?
Mandatory for ALL associates. You can apply for an exemption due to moral/religious/allegeric reasons, but it has not been granted for many. This is the first year that is has been deemed mandatory.

How Can I become Stroke Nurse Certified?
The CNRN covers all aspects of neurological nursing, not just stroke. You can take the NIH Stroke Scale cetification online for free through the American Heart Association.

Ever had a patient or family members fire you?
I've been fired and it was no skin off my nose. This family also fired two critical care intensivists and almost every other nurse on my unit. So we got ethics on board and the situation snow-balled from there.

To specialize or not to specialize...
I started out as a nursing tech in a Progressive Care Unit. It was a great base to learn skills, but I knew that I wanted to be an ICU nurse from the moment I started nursing school. I was able to obtain an ICU nurse residency position during nursing school and accepted a job on that same unit once I graduated with my BSN. It was a great experience, but I soon discovered my passion for neuro ICU patients. Once I moved to Chicago, I accepted a job in the NSICU at NMH and never regretted a moment! My specialty has opened many opportunities in the nursing community from medical device education to patient safety. Go with what you love and you won't be disappointed!

CRNA or Other Specialty?
I would *highly* suggest more that 1 year of ICU experience. You will be administering anesthesia to patients during surgery. This is not the job for those who are just looking to make $$$.
