"it's about me and my practice" Excellent!! That's exactly how I feel too. Although I'd feel this way no matter where I worked. But, behind the wall, it really brings it home especially when I take care of an inmate hospice patient. They are ...
I think this is great. However, at our local hospital DNR wishes are suspended during surgeries. The only reason I can think of is that the surgeon doesn't want that on his statistics. Also, usually DNR orders are reviewed prior to surgeries. ...
8 months ago our new DON mandated SOAP notes. We were recently told that part of our yearly review will include chart reviews of our notes. I don't believe this type of note fits every situation. Does anyone else have to write SOAP notes?
Wow...."north of $170???" combined??? But your take home is more than his? You are paid well. In what state do you work? I'm in the northeast. Cost of living is high. I make $60K without OT and I've been a nurse for 5 years.
I was the first medical person on the scene of a single motorcycle accident on July 17th. I have experience as a first responder due to my correctional nursing experience . The driver was laying in the middle of the passing lane and the motorcycle wa...
I have been in a mens' correctional facility for 3 years. I constantly hear other nurses making judgements about an inmate because as one of them put it today, "they are always out for secondary gain." As a result, inmates whom may have real medic...
I'm an RN working in a state prison system. I'm very interested in getting my certification in wound care which would benefit the prison system as a whole. We have patients who go to a wound clinic at a local hospital and they come back with wet t...
I'm applying at Wellpoint for a case manager job. I've read mixed reviews online about this company as an employer but that was from all segments of their workforce. Nursing opinions are what really matter to me so I'd appreciate any opinion you m...
I used a non re-breather mask with 5L O2 with a Pt who was mottled, RR 28-32, O2Sat 88. I couldn't find a regular mask and my thought was that he was mouth breathing so a canula wouldn't have done much good. Did I use the right O2 delivery system or...
Thanks for the tip about taking off the rubber tabs. I work in a facility with minimal equipment and we have to work with what we have. I had looked for a simple mask but couldn't find one.

What a fabulous opportunity for you.. Many new nurses find that their first jobs just aren't the fit for them.. A trip to Africa? I'd take it in a heartbeat.
Hi...could use some advice... Today I went into a patient room and discovered the patient with a respiratory rate of 28-30, O2 Sat of 88. In my note I documented it as dyspnea. I got home and now I'm thinking this......Should I have used the word ...
Thanks for that. One of the nurse's on that second shift said to me, "we'll take over from here. You did the best you could." I really needed to hear what you said, "awesome nursing assessment."
Hi....I stayed over into the second shift to help out because they were short. While delivering 1600 meds, I discovered a patient who needed assessment immediately. Did my assessment and the 2nd shift nurse strolls down and I told her what was going...

In my experience, the ADON job is multi-faceted but maybe in such a large facility, you'll only have one hat. That is the first question I'd ask - will I be the Staff Educator, Infection Control Coordinator, or the Rehab Nursing Coordinator? In smal...

Pt asks for coffee for the 13th time during a time when you're down 2 LNAs and Pt down the hall is in respiratory distress. "You're all set" "Do I look like a waitress to you?"
Orion...thanks for your comments. You answered my question! This was abnormal for the patient. Extremities were mottled, Pt was almost panting, and very lethargic. First thing after the visual assessment was VS, then grabbed the O2. Then the s...
So, I work at a facility where the other nuses pre pour meds for the 2nd shift, including narcotics. I seem to be the only one who won't do it. I'm really feeling the Love from my co-workers for this....NOT. Any advice is appreciate.
