Anyone have any info about this drug? Im having trouble finding anything thats directed at healthcare professionals. Have heard from Hem/Onc that the ingredient that inhibits thrombin is, or is similar to, what is in Leech saliva? Cant find much about monitoring PTT/PT, bleeding risks, half life, etc... I know that Argatroban has an effect on PT/INR that causes a little trouble when you are transitioning onto coumadin, but Im wondering if this drug is similar? Anyone know?

Anyone live in Highland Creek and want to give advice (good AND bad) about this community? The husband has decided that we are moving and I like the homes Im seeing online in this community.

Its legal in AZ. The hospital down the street from me has a drawer in the ER where the baby can be placed and when its closed the baby is inside the hospital. Ive talked to several nurses who said theyve had babies dropped off there and it works out really well. Of course then they go into the "system" and thats a whole different story. But it beats hearing about them in the dumpster. Especially in AZ where right now its 113 degrees. I thought that when the mom dropped the baby off like this she gave up rights. Must be different state to state. Anyone know?

Did she go to Scottsdale Shea? I work there and I took care of a stroke pt that came from PV. I wonder if its the same one?

Thank you all so much for the responses. I was just debating this again today, even considering going to a Trauma ICU, but I think I will stay at my hospital (community) and go to ER. Sounds like trial by fire but theres nothing wrong with that! I love a challenge. Thanks again!

Well Im thinking its about to time to get out of Med/Surg. Long story short....its been 5 months and Im bored. I have SOME MICU experience as an RN (I was a new grad in a new grad UNfriendly enviornment) and some ER experience as an Extern. Anyone working Trauma? Anyone have an opinion regarding going to ER then moving to a Level 1 or just jumping into a Level 1?

Just be careful that they cant get you for abandonment. Im not sure what the rules are in WA, but in AZ if you refuse to take report you cant be accused of abandonment. If you arent sure, check out your states regulations.

Someone said on here one how "disheartening" it is for a preceptor to put all her efforts into a new grad in the ICU just to have them leave. Well I say how much MORE disheartening is it when a new grad comes to ICU with so much intelligence and ability, ready to soak it all up, just to have all of her hopes crushed because of poor staffing, absent management, lack of proper orientation, and burned out preceptors? And to think this is your first true nursing experience! It is so not worth it.

Everything you learn feels so overwhelming, but if you can just let yourself relax enough it will all filter to the surface when you need it. I was on my way home from a camping trip a few months ago and we pulled up to an accident that JUST happened and a young man was laying unconscious on the side of the road in the middle of nowhere bleeding everywhere. He was surrounded by about 10 people who were just staring at him. When I walked up to him the girl at his head said to me "he just lost his pulse" and I knew exactly what to do. It was like an instinct. You just have to trust yourself.

Did they give you the correct answers to these to check your work?

I am not a hospice nurse, but this is something that comes up more often than I thought it would in acute care. I would like to know what your explanation is. I know this a huge grey area for me and I would like input. Thanks.

Im probably being an inquisitive idiot and forgive me if this is a silly question but, can someone please explain to me the difference between a tunneled and non tunneled CVC? And does anyone do cutdowns anymore?

I went straight into MICU as a new grad. I lasted 13 weeks and then decided I wanted to work the Frosty machine at Wendy's. I quit and spent the next 5 weeks on my couch. And it wasnt because I "couldnt handle it" or wasnt smart enough, I graduated in the top of my class. I even did my preceptorship before graduation in said MICU. It was INEXPERIENCE. I had a renegade preceptor and an absent manager. Which is fine if you have some experience. How can you feel like a competent nurse if you havent started an IV since 4th semester and every single patient has an A line? How can you learn to organize if you have never had more than 2 patients? How are you not going to totally FREAK OUT when you forget to flush the mediport after drawing blood cultures and it CLOTS OFF:chair:!?!?!How are you going to handle terminal weans if youve never taken care of a dying patient, or better yet, THEIR FAMILIES, ugh! Anyway, after 5 weeks of festering I went to med/surg (in a different hospital) and Ive been there for 5 months and I feel ready to go back to ICU. You probably dont want to hear this, but my honest opinion is dont do either until you can handle 5 patients and can cover an LPN and still manage to keep it together when one of your patient codes. Please dont take this offensively, just trying to offer my experience for you to consider when making your decision.

The way I understand it is that serum CO2 is actually in indirect measure of serum HCO3- since approx 90% of CO2 is present in body and bicarb. Remember that serum CO2 is venous, and PaO2 is arterial. Their ABG is probably fine if the sat is wnl and they arent in distress.

oh my gosh im so excited about this topic. these are really great questions! first of all, i am an witness and an rn also. i am soooo glad to see that there are so many of us on here. i would love to get to know you all. its good to have the support. as far as on pump cabg goes... yes, technically the blood leaves the body, but a lot of witnesses view the pump as a temporary extension of their circulatory system since the integrity of the system is never actually broken. there was an article in the awake magazine about this in 1989 (i think it was 89). it boils down to this: the bible commands us not to "eat" blood, which would obviously include iv infusions. the bible does not specifically mention things like: on pump cv surgeries, albumin, cell savers, dialysis, etc... these things are left to the patient to make a personal decision about. they have to consider their understanding of the scriptures and they have to become educated regarding the treatment modalities. then they make a decision based on their own conscience. this is why you will see some accept one thing that another would refuse. hope this helps.

Hi... This is my first time posting anything. I currently live in AZ. We are SICK OF PHOENIX and have just about decided to move east. Im still fairly new to nursing (graduated in 2005) and my goal is to get into Trauma and Flight. We are thinking of moving to the Charlotte area. I need info regarding Level One hospitals and/or teaching hospitals in that area. Also, my husband builds hospitals (he's a Super for a Commercial Builder, healthcare division) and we would need to be where theres lots of commercial construction. He likes to build big things. We have a 2 kids and kinda want to raise them not DIRECTLY in the city like we are now. So Im looking for a place where we can both work and where our kids can actually play outside without my constant fear that they are going to be hit by a car or kidnapped from the front yard. Is the Charlotte area and surrounding suburbs where I should be looking? I have family moving to Wakeforest, whats that like? And he's all worked up because he's heard the humidity there is a killer. Anyone have a comment on that one?
