Easterners, Waterbirth?
at our birthing center they sometimes call the tub an aquadural!our patients labor and birth in the tub[low risk]. most enjoy some cant stand it . they are helped out before the placenta is delivered. for some couple the dad may get in also. we have a few videos from the center that we show to our clients who are curious about it and show them the video[with the couples permission who gave us the tape to use] most moms are glad they used the jacuzzi and think they couldnt get by without it.

When circ's go bad...
I just recieves a dvd from no circ .org about circ. only about 50% of our babies are circed and some ask what i think .of course i tell them that it is an individual decision, and just because daddy is doesnt mean that the baby has to be. if the child asks why his member looks diiferent than daddys he can tell him that when he was a baby they did it all the time, but now we learned that we dont need to do it so you look like this and i look like that!

Room for Guys in this specialty?
I work at a cnm practice and in the beginning some of the clients objected to a male midwife, however once they meet him there is usually no further complaints. Some even prefer him over the female providers. Good Luck

Rue Education
I used rue ed. it was expensive, but you could make payments. Also they are not related to ec so you have to be careful of the info /advice they give you. They had told me not to register at ec until i was ready to take the cpne,so when i did i lost 3 nursing concept courses because ec changed the exams and if i was enrolled they would have let me keep them.BE CAREFUL,they can be helpful i got mostly As

I have been asked to be a Doula!
Encourage your friend to write a birth plan, with her wishes hopes and desires.Example DO NOT OFFER PAIN MEDS.I KNOW THEY ARE AVAILABLE AND WILL ASK IF DESIRED. etc.

cloth vs disposable diapers
Some of our clients like a brand called fuzzy bunz (?sp). Some just buy enough to get started and the crafty ones use them for a pattern.

a male midwife??? :S
I work with a male midwife, and he has a soothing touch. Some clients prefer not to see him{usually the sexually abused or for religious reasons} but we cannot guarrantee whom they will have in labor. Some who were opposed to a male midwife come to see him for their annual exam by request. How the midwife treats the client is more important than the sex of the provider.

Newborn death
Thanks to all who have replied. Each day does get better,i am sure it will be harde when they come for the 6wk pp visit.Moms TORCH was neg[we thought it would be]. We are awaiting the autopsy results,i think once i have answers maybe it will help.I do think that slowly it is making me a better nurse because i can empathize more. My husband says that i was due to have a loss/poor outcome and I was lucky it took so long.I greatly appreciate everyones support.

Newborn death
Suffering loss as a professional for a baby who passed away at 36 hours at er.I did the pp care and knew the family from their previous birth experience. Did a home visit the next day, after the passing no cause of death . I have been a nurse for23 yrs, i accept that things happen however having been in ld/pp/newborn care this is my first personal lossof infant. I have grieved on and off, very little with other staff members.I went to the funeral, very difficult, but i am positive that it is nowhere near the loss the parnts feel. The prenatal course was totally wnl,as was the L/D/pp and newborn exam. So I am like did i miss something,/ i continue to do this job?Husband is supportive but I am not sure he gets it.I was holding the baby one day and the the next day i get a call that the baby died!!Even though this was not my child,i feel like i am going thru the stages of grief. How do i know if this over reacting or genuine sorrow that i must work thru??Any suggestions?

For L&D RNs who are future CNMs: Coping with different model of care??
Try a freestanding birthcenter. It is wonderful for the right candidate, they usually do not pay as well as the the hospital because they are ususally self supporting. By the way we are usually reimbursed by the insurance company at less than half the rate of a hospital delivery. 99%of our moms breastfeed, use the jacuzzi for comfort measure and for tub birth. A wonderful book to read of birth stories is spiritual midwifery by Ina Mae Gaskin whom actually still practices on the FARM with a group of midwives with wonderful statistics. They also have teaching clinics at different times of the year. Our one midwife says that just because they hang wallpaper and add a plant at the hospital does not make it a birthcenter. One last thing, our midwives join our clients when they go into labor at the hosp or bc and stay with them until delivery, they dont jusxt popin to catch at the last minute.[unless they come in that way!!] Hope this helps. :)

Taking an infant's temp.
I work in a birthing center and we always do axillary from birth to d/c usually 6 hours later. We tell the parents to take the temp under the arm unless the md asks for a rectal.
