Please give words of wisdom. ThanksC. Previous are plans and revising goals don't matter anymore. They have had surgery so things may be completely different than before surgery. The first thing you would do with a patient returning from surgery is assessment, including VS.
Nov 2, 201312 yr Scratch that. The question asks you what must take place BEFORE modification of the care plan and that could only be assessment. The other answers don't work with the way the question is worded.

Nov 2, 201312 yr Author Thank you everyone. I way over think things and I talked myself out of the reassessment for various reasons which now seem so stupid. Some days I am so over whelmed with everything and I doubt that I can do this. Sigh...and this is just Nursing 1. LOL. My first care plan is due on Tues and it is freaking me out. Nov 2, 201312 yr remember you must assess before intervening! in addition they are post-op, so they have undergone a change that needs to be evaluated. Previous plan of care was pre-op, they are now post-op :)

Then the process begins again with assessment. So I would pick A

When I'm dot with a test, I go back over it to make sure I have filled my scantron out correctly but I meet change answers. I changed 6 answers on a test once and missed all if them. My first choice had been the correct answer the hole time.
