Hi I think I am going to like my job. I am working in a facility with only 39 units. The clients have Alzhiemer's dementia. The other clients are assisted living clients. I wish other facilities would have this kind of system. I don't believe Long term care should have so many people in a building. The care they get is so very sad. I don't believe families would want that for their loved one.

We probably did give them I just don't remember.

I just got a job in a assisted living facility. They say it is not skilled but I think it is. I have questions. I wonder what a bipap machine is and how it is different from a Cpap machine. I worked in a facility once before and I had a lady who could not breath. I sent her over to the hospital and they ended up putting a stent in. Is that what they do for Atrial Fibrillation. We never gave her her medication when she returned from the hospital. It seems to me they would have put her on Coumadin though. About two years later she had a stroke and could not talk anymore. Her family discharged her and took her home. I just wish I could take better care of people. I hope my story makes sense. I am also looking after people with Alzheimer's dementia. Does that disease affect breathing rapidly or can people develop other diseases like Atrial Fibrillation more often before Alzheimer's Dementia complications occur? If anyone would like to shine some on these subjects, I would greatly appreciate the help.

That lady left or was fired. There was a high turnover for that position. One of DON's was escorted out of the building.

Gosh you guys are rough on me. I would like to stay at the place I will be going to. They really need people to work there.

My director of nursing at an assisted living facility told me to argue with the psychiatrist over over a drug change. We had a lady who clearly had sundown syndrome. She was on Paxil. He told me that he wanted to change it it Zoloft. I did not argue with the man and he put her on Zoloft. I was not reprimanded for his decision. The Director of Nursing told my friend that she did not have to have her blood drawn when she stuck herself with a needle after giving an injection. What gives???

I have been reading the book Mastering Geriatric Care. Are there other clients recovering from surgeries in nursing homes today? The reason I ask this is because I had a friend recovering from bowel surgery. She lived alone as was recuperating in a nursing home.

I am working for Maxim as a per diem nurse. They are going to assign me to various nursing homes in my area. What is the charge nurses responsibility on any particular shift. Are you supposed to tell them when someone is in declining health? What exactly do they do during a shift to help? If you decide to send someone to the hospital do you have to tell the doctor in charge of the patient? I have also been told that you should try to keep the client in the home and not send them out. Do doctors give orders for dressing changes or do you ask the wound care nurse? I am pretty nervous.

I am an LPN. I have not had any action taken against my license. Two years ago I lost my home care case for some reason. I really don't know why his mother decided to fire me but she did. I do know that the nurse that came into replace me decided to have a tracheostomy placed. This was after a spinal stenosis surgery that helped to fix a collapsed lung. I have no idea why the tracheostomy was placed other than he had a lot of upper airway congestion and he hated to be suctioned. Anyway after being fired I had some money saved up so I took some time off to be with my son. But I want to go back to work because I feel it make me happier. I don't know if I can. The agency I worked for wanted to give me another case but then they decided to fire me. I am really not sure why other then they thought I would not be able to handle the cases that they had. They were all premature infants at the time. I have had a case like that in the past that we sent to the hospital for a bowel intussception. I don't know what to put on my application. I have been fired by both the agencies I have worked at even though I have worked for them for so many years. I really love nursing and want to help. It is very time consuming at times. It is hard for me to find a balance between my home and work life. Any suggestions would be helpful. I have thought about being a pharmacy technician but I don't think I could pay my bills. My husband is not a bread winner either.

I took care of a teenager who I believe was like the way she was because her mother drank during her pregnancy. She had scoliosis and was on a ventilator by the time I was on her case and her legs where very contractured too from her seizures. She loved to play with her marble run and liked her baths.

I do over chart I think. Every line of my nurses note is filled out. They say it is Exception charting. I just don't understand how my charting relates to the nursing care plan.

I am an LPN and I have worked in home health for a long time taking care of sick children and disabled children. I have several questions. I often feel frustrated with the "medical care plan" that my home care company puts out. I would like to write own care plan for my client. I think it would make my job more interesting too. I know that is not a question and it is just a statement. It is hard for me to always finish my charting at the home. I usually get the assessment done and some of the nursing documentation. I always feel timid about this issue when I want the family to sign my chart. Should we leave the documentation in the home? Another thing about home care nursing is that it would be nice to make friends. I usually work alone. I get so down in the dumps because I don't get to talk to the other nurses. The cases I have received are children who need total care. I am not always sure that I am up to the task. I do know when something is wrong though. I never get to talk to the Director of Nursing and I often embarrassed to do that anyway since I have such a hard time with the documentation. Anyway so far these are my thoughts. I often document right out of a textbook too.

When I was a care giver we had a couple move into a tiny little room so undeserving after a marriage of fifty years or so. The nurse on the previous shift had given him a stool softener and he was able to go. But instead of putting him on the bed pan for some reason we got him up to go. He deficated as he was standing up and slipped in his mess. I came in the next time and he was gone. He could have broken a hip.

I have not read all these comments but I think by law the cardiologist is supposed to follow them. I had a little girl with down syndrome. Her family told me that.
