by the way lovingtheunloved, it's not very "therepeutic" to refer to the OP's sibling as "crazy". It is no longer correct to use the "R" word, make jokes about short busses or gay people, but somehow it's still acceptable to marginalize people who are overweight and people who suffer with mental illness. Are you thinking about a career change?

Thanks everyone for your help. Sorry to have such difficulty verbalizing my question. And you are right, I know that pharmacology, patho & phys are huge bodies of knowledge that I dont expect anyone to cover here. I think I am missing something in the bigger picture of the pathways and will take your advice dirtysleepynurse and choose a drug and follow it that way. I understand the way drugs enter cells etc. To take an earlier example of an infected great toe, lets say we hang an abx. How does the drug "know" that the big toe is the infected place?

This is a goofy question, but I learn best when I can visualize a process. So here goes. I want to better understand how a med administered parenterally actually works. For example: IV med into vein/deoxy blood moves to heart/lung circulation, systemic circulation, tissues and cells of the entire body. How does the drug "know" where to go to affect the desired effect? I understand first pass etc. I am asking about meds going to work at the cellular level. Thank you for your help.

Assisted living center says dad is having an affair
What is the reason for the notification of the wife or competent party? Is he at risk for putting other resident's in harms way? Is the assisted living facility informing the family because they have some liability? Would the facility also inform the single lady's family?

Here'e the situation: The assisted living center where my friend's dad and mom live (in another state) called today to tell her that her father was found in bed with another resident (unmarried woman). They explained that were calling becuase it was their ethical obligation etc etc. and that sexual activity between residents is common and they were considering changing her fathers meds and were going to discuss it with his doc. My question: Please explain why the assisted living center felt obligated to share this information with her? Would'nt that be considered a HIPAA violation? My friend is the medical POA, so maybe they needed to tell her about the med change?

tips on how to survive 12 hr shifts.
I am a student and just started clinicals working two 12's in a row. It's tough but the time does fly. I agree with everyone else, hydrate, snacks, shoes, arch supports and compression stockings. You can get them at a med supply store w/o prescription. It will be the best $25 you have ever spent. Beware, they are sized by shoe size, not by calf size etc. Get an experienced salesperson to help you bc you usually cannot return them if they do not fit. More than anything, the sox have saved me.

During OB rotation, (last day was yesterday). On my first day, I put a diaper on an infant backwards, noticed my mistake and fixed it. I don't have children and havent changed a diaper since babysitting in the 60's. During assessment of mother, spent "too much time" - wanted to show the instructor that I gave a thourough evaluation. A list of 8 problems were written up and forwarded to school. These issues will be passed along to my next clinical instructor. Class starts Tuesday.

Anyone ever been dinged by clinical instructor for minor mistakes while on clinical rotation? I have been written up and it has effected my confidence about going onto next rotation. I have a sick feeling in my stomach all the time, I've lost my appetite and food goes right through me. I just want to cry!

My 9 year old patient was raped and required surgery
I agree with those who have suggested that a team approach to her care would be best. I would consult with a SANE nurse as well as social worker/therapist/psychiatrist who are experienced and trained to work with children who have suffered similar circumstances. I agree that for your own care that you consult with someone about your past issues. There is a time and place for therapeutic sharing of self, but I don't think that this is it. You know too little about what she actually experienced. At this point attempting a hug could frighten her and may be misinterpreted. I would use caution and not touch her outside of the care that you provide. Document everything. Your consistency will help her slowly adjust. Give her your heartfelt smile and your gentle voice. Let her guide you. She may have little to no experience with caring adults.

Oy - and my heart - I forgot to tell you about my heart. The CV s/s are endless and ongoing. Angina, SOB, fluttering, stabbing pain, 2+ bilateral pitting edema on LE. I think I may be having an MI as I am writing this.

Don't feel bad. I was with friends at dinner recently and we were discussing vacation plans. They: SCUBA trips, cruises, south of France, Italy, Spain and NYC. Me: a trip to the Mayo Clinic where I could have Dr. House run a full battery of tests, scans etc. After a full head to toe, I want the treatment. Everything that medicine has to offer - in addtion I'll need group therapy, weight reduction plan, forced exersise, piano lessons and a drawing/painting studio - oh, and poetry readings. Yes, I am sure that all this will cure the MS, stroke, multiple tumors, detached retina, Parkinsons, cancer and hang nails that I experience on a daily basis. AHHH.... I feel better just thinking about it! p.s. I'm only a nursing student and am certain that when I start practicing my diseases and syndromes will only get worse.

I have a suggestion that may help answer some of your questions. See if you can get the origal tests (and your answers) that you took during the semester. Ask your advisor and or instructors to go over each of the exams with you. Pick apart the exams to determine the types of errors you are making. For example, are they knowledge errors? Are you having difficulty synthesizing and applying your knowledge? Are you having difficulty in critical thinking? Are you reading the questions and making sure that you understand what the question is asking? Do you know how to find the "stem" of a question? Are you overthinking the question? Are you running out of time during the exam and rushing? Are your errors related to test anxiety? Also, you may want to check with your school to see if there are any tutors available that can help you as a learning coach. I think it would be beneficial to review your mistakes and find any patterns in your performance.
