While someone can teach things they don't know, or haven't done, how exactly are they able to answer the questions asked by students, or teach effectively? This is one of the biggest problems I see in health care education these days. And we wonder why we have students, and new practitioners, that have problems performing effectively in the patient care environment.

As a Paramedic of 29 years, and 28 of those working in a city that regularly ranks in the top 5 (many times top 2) of the most dangerous cities in the US, I can tell you that if I was "off the clock" I would have called 911 and reported where it was and what I saw. So no, I would NOT have stopped. The thought to pray was fine, but please call 911 FIRST, then pray.

Good question and if you are really interested in learning more about the treatment of VF & pulseless VT, you'll find that there is NO evidence of any drug, improving survival to walking out of the hospital neurologically intact. Also, remember that any antiarrhythmic can also be proarrhythmic. Also note that while there is some evidence of short term increase of ROSC in VF/VT patients with Amiodarone, there was no increase in those patients walking out of the hospital neurologically intact. The only treatments known at this time, that improve ROSC and walking out of the hospital neurologically intact, are high quality, uninterrupted CPR and defibrillation.

I use the idea of traveling down a highway (for my area it would be I-95) and you come across road construction and the travel lanes get restricted from 2 or 3 lanes, to 1 lane. Thus the traffic backs up and slows the movement thru the area. And then when an MI occurs (complete blockage/ an MVA) the idea of how you would try to get off the highway and look for an "alternate" (collateral vessels) means of travel, via side roads, etc., to get around the closure of the highway. Very simple, but usually seems to work when trying to explain. Hope it helps

Let there be NO mistake, taking and getting an ACLS &/or PALS card does NOT mean that you can intubate. Why anyone would think they can, is still beyond me. As an ACLS & PALS Instructor for well over 20 years, I will tell you that you are mistaken, if you think they mean that you can intubate. This is one, of many, reasons that the cards now are for course completion only.
