How can we make the process of report from ER to floor better?
We don't tell a squad to wait. We move the now stable pt into the hallway if we need the room.

How can we make the process of report from ER to floor better?
I have worked on the floor and now I work in the ER. We have a policy that no patients can go up to the floor during shift change. So for us, no transport or giving report of pt's between 7am-8am and 7p-8pm. We hold them in the ER til 8 then send the patient. We call and give a verbal report, if the nurses isn't able to take the call, we wait about 10mins. If the nurse doesn't call you back in 10min, then the patient goes up. We have the same system so the nurse can look up everything in the computer. If they have questions then they can call me back and I will answer them. All ICU/PCU pt have to be taken up to the floor by the nurse, so I can give a bedside report then.

Mandatory flu vaccines for staff
I am a nurse, and I got Guillain Barre Syndrome from the flu shot many years before I was a nurse. I have never gotten the flu shot since I have been a nurse, neither have my 3 children. We haven't had the flu in our home yet. We wash our hands. I'll take the flu ten times over before I will go through the 6 months of hell I went through when I have GBS.

Wanting to quit orientation, new grad, please advise
The first step is to speak with Nurse manager and ask her for a new preceptor. The preceptor you have does not necessarily know what she is doing just because she has been there for 20 years. That nurse gives the good nurses a bad name. She has no respect for her ancillary staff. The cleanliness of your patients is "YOUR" responsibility. The ancillary staff work with you, but the care of that patient fall on YOU. They are not going to call the CNA into court when they get a pressure ulcer, they will call you in. Moving on to copying someone else's assessment... I can't even believe that she hasn't been caught doing that. That is totally unethical and unprofessional, not to mention dangerous. She is lucky that she hasn't lost her license. Please don't pick up her bad habits. There is nothing worse than following a nurses that doesn't want to be a preceptor. I have been lucky that I have alway had people that wanted to teach. There was one day when I didn't because my preceptor called in sick. They placed me with one of the more senior nurses. It was the worst day. I went ahead did what I knew was correct and then spoke with my manager and the charge nurse as asked that if this situation happened again that I would prefer to be placed with another nurses. I just told them that she appeared not to be interested in being a preceptor. There were plenty of other nurses there that were competent enough for me to follow. They said that it was not a problem and I didn't have to go into too many details. I think the delivery of what you are saying needs to be professional and respectful. It is not your job to oust the bad nurses (at least not when you first step into the facility), but it is your responsibility to preform you duties within your scope of practice and provide safe care to your patients. Has your manager asked you how things are going? She should be checking in with you on a regular basis. If you don't feel that you can approach her call the Nurse educators and speak to them. Both my educators and managers told us to come to them if there were any problems. And you should. You have worked too hard for your license to lose it over a lazy nurse. And trust me, that nurse will not back you up if for some reason, something comes up while you are following her. She will throw you under the bus. As a new nurses you also need to learn to stand up to other nurses as well. Your preceptor is a good person to start with. I would question her on why she thinks its appropriate copying someone else's assessment. Find a patient where the assessment is different and prove her wrong.

Best Nursing Shoes/Sneakers
DANSKO by far. Prior to becoming a nurse I was a flight attendant spending up to 18 hours a day on my feet at 35,000 feet where most people's feet swell. Dankso's are not only comfortable but they are good for your feet/back. They provide good support for the arches. I have painful bunions as well and I can wear these shoes all day and my feet never hurt. Make sure if you get them that you are fitted properly. They do not fit like normal shoes. They are a clog with a back and the heel of your foot is meant to lift out of the shoe. The shoe should be a loose fitting shoe not tight. They also last forever. I have one pair that are over 10 years old and still look great. I wear them in my every day life as well. They come in every color imaginable. Not the prettiest looking shoe, but worth every penny you spend on them. When you first put them on they feel weird. They are heavy. But wear them for a day or two and you will be hooked. If for some reason you don't like them, they will take them back. Great company to deal with.

The Patient I Failed
Wow. I won't forget this. Wish I could make a copy of it and give it every family member that insists on not listening to the wishes of the patient.

Job offer on the spot
They should give you the opportunity to discuss it with significant people in your life. At least one day. Then I would cal your 1st choice and tell them that you have an offer on the table but that their hospital is your 1st choice. Is there any way they could give you a decision.

Lost my dream job
Please don't beat yourself up. You are not stupid. What is stupid is that you passed the same test as a BSN nurse yet they feel that a BSN nurse is more qualified. What I find in my hospital is that the ADN RN's are better prepared on the floor then the BSN nurses. But if BSN is what they want then you give it to them. Enroll and call them back and tell them you are enrolled. Many places will take you as long as you are enrolled in a program. I got offered two jobs where they supposedly only take BSN RN's, you need to reinforce why you are ready for the position. It is all in how you present yourself. I believe advancing you education is always wise . This is an ever changing career. It is a must. Look at it as an opportunity. There will always be another dream job and a better job.

Our new hire, new grad charge nurse
It's funny they did that at my hospital with me. I am ADN RN and I was not even off probation when they asked me to be in charge along with one other BSN nurse. I was so caught off guard when they asked me. I asked her if I could think about it. When I thought about it, I realized I worked way too hard to get my license and I wasn't about to lose it over rushing through my duties so I could get some extra paper work and CNA assignments and RN assignments. I told my boss that I was flattered and that in time I would be up for the challenge. Right now my priorities are to my patients and becoming the best nurse I could be. The extra $24 a shift was not worth it to me. I help the charge nurses out when I can so when the time comes again, I will transition easily into the role. And I too thought they had lost their minds in asking me. I think I am a good nurse but I M still learning so much everyday. I think that new grads that take the role too soon are playing with fire.

Am I over-thinking, or are these just bad test questions?
First off nursing nclex questions are a breed of their own. I so vividly remember being in your shoes. Knowing now what I know, all the answers are correct and I get them correct without hesitation.(that is because I was answering those questions for two years. You get better at them the more you do practice question. Some of those question were straight definitions, so you may need to read your book to make sure you know what all the words mean. In your freshman year you learn so many new terms and it important to have a clear understanding. Most nclex question will not be so easy. Those question, I hate to tell you were on the easy side of nclex and you will not seem them that easy on the boards. Glad to hear they are easing you into the questions. My suggestion is to to the question book Fundamentals Success question book. Do questions out of that book in your spare time. ( I know there is Jo spare time in nursing school) I worked full time, had 3 kids, and school full time at nights and weekends. The free time was taking away my sleep. On average I only slept 4-5 hours a night for the 2 years I was in nursing school. It is hard and it is new way of questioning and thinking and you need to train yourself to think like nclex does. It take time and a lot if practice. Hang in there and make sure what ever type of questions you are doing, read the question at least twice and ALL the answers twice as well before answering. When you get them wrong go back and read rationales. Don't ever buy any nclex book that doesn't have rationales. My favorite books were the Saunders question books and the Success question books. So our teachers took the question right out of the book. Good luck

VENT: school still ruining my life after graduation
I keep reading things about CA.. They are pretty screwed up there. In NY you can't even register to take NCLEX until they recieve info from the school that you have graduated. You can apply for licensure and send all your information in before you finish, but it stays on hold until they recieve the offical info from the school. So I had all my stuff in at the beginning of December and the school sent the offical notice at the end of December. I got the authorization to test via email and scheduled for the end of that month. Two days later got the results.

VENT: school still ruining my life after graduation
I never really had anything good to say about school except I did make some of the best friends there. I still remember my first day thinking this is going to be the worst two years of my life. But thank God for my study group or I wouldn't have made it. Of course, now I can't get a job. I passed my NCLEX (in fact I was the first one in my class) I have sent resumes out, called, stood on my head... But no job prospects. I don't even know what to do now. Of course my school is no help. Every hospital that I did clinicals said they they had budget cuts and are not hiring any new graduates. Figures... just my luck. Now I am trying to beg every person I know at any hospital to bring my resume in for me. Somethings got to give soon.

Pearsonvue Trick Is this TRUE? Does it work every time?
Congrats!!!!:yeah: to everyone that just passed. I just got my actual license in the mail. What a nice feeling. Now if I could only get a job. The job market in the NY city area really stinks for new grads. If anyone can help me with that let me know. I live in Westchester County and I am willing to go to CT or to NY city for a hospital job.

I can't imgine that long because I already have Chemistry and Statistics.
