MRSA & sex
Ok. I'm doing NCLEX-RN practice tests. One of the questions was about a pt asks the nurse when to resume sexual activities after being d/c to home after an MI. So, this prompts me back to a question a pt posed to me and a doc. when I was a nursing student in school. The pt had MRSA and asked what he can do to not infect/colonize others when having sex? The doc didn't know and neither did I. And, I was caught up with other stuff so I totally forgot about this until now. Any thoughts?

flail chest - pneumothorax
NCLEX-RN practice question: Nurse should plan to care for a pt with "moderate flail chest" DOES NOT include: pleur-evac drainage system with the rationale that pleur-evac drainage system is for pneumothorax. From Brunner and Suddarth's Med-Surg textbook (see link below), pneumothorax can be a complication of blunt trauma (which is the cause of flail chest), so why is it that the nurse should NOT plan to care for pleur-evac in this question? Med errors: subpoena, copy of incident report
Why can't a nurse make a copy of the incident report (of the medication error the nurse made) as a personal file? Yet, she can keep a writing note of the incident. The NCLEX-RN rationale is that the nurse can be subpoenaed in court ? (it's a NCLEX-RN question and rationale that I don't understand). I don't understand? Help would be greatly appreciated

Physical exam for a High school student
OK. One is a BSN, RN and his sister is a high school student. His sister is in need of a physical exam to submit the results to her high school. She asks him to do a general physical assessment report so that she can turn it into the high school in time. Is it lawful to do so?

Pneumocystis jiroveci pneumonia
Practicing NCLEX-RN: How come Pneumocystis jiroveci pneumonia only requires standard precaution? I thought all pneumonia requires droplet precautions? (remember SPIDERMAN mnemonics for droplet precautions?) I did a search on Google on the mode of transmission of Pneumocystic Jiroveci but couldn't locate a satisfactory answer and a trustworthy source. Any help would be greatly appreciated :)

Liver biopsy post-op and R-side lying
Again, I'm studying for NCLEX-RN. Even tho I've googled and learned that R-side lying after liver biopsy helps because it creates the pressure needed to counteract bleeding as the liver is very vascular. Still, I don't get it, I thought gravity will drain even more blood out? Thus, it should make more sense if the pt is positioned on L-side with appropriate pressure applied? Any comments, please? Thx :)

Leg cramps = Blood clot?
Oh, I think I've got the answer. Blood clot may cause ischemia. Ischemia may cause leg cramps. Therefore, when pt experiences leg cramps, there's a possibility that there's a blood clot. Agree?

I'm practicing for NCLEX-RN. There is one explanation for 1 answer that doesn't make sense to me: Cramping in the calf = blood clot possibility (the pt is total knee replacement post-op). I've googled it all but couldn't find how cramping may indicate blood clot possibility. Physiologically speaking, cramping sensation results from the spasm of the muscle. Blood clot, in my novice understanding, does not cause spasm? So, if anyone could please explain to me? Thx!!!

I'd suggest you do some research on how other ethnic groups like blacks, asians, latinos, other religious views look at diseases and view their lives in terms of pain, time, life, family cohesion, autonomy, informed consent, freedom. Then, you'll realize how different you are. That will help you answer the question. I hope it helps.

Is it okay for a CNA to call herself a nurse?
Here is my 2 cents. I think CNA's do a lot of nursing work: bathing, taking care of patients, comforting them, some even spend more time with the patients than RN's do, etc. I remember I attended a talk by a nursing leader and he said that when we delegated our bathing pts and so on to others, we've given away our nursing notion. I feel like CNA's are even more nursing than nursing RN's. So many RN's dread the thought of taking care of patients' bodies. So, I think your friend probably just need to clarify with the patient that she is an RN whose scope includes some other things that a CNA title is not entitled by laws to do. I hope it helps ease the tension. just my 2 cents.

That is very helpful. Thank you for your advice. I'll discuss it with my preceptor. She's very tactful with me and always kind to me. So, I'm thinking that she won't tell me what I'm doing to her. I think I should just go ahead and let her know what she would want me to change. Thank you very much.

Hi, Thx for all the good pieces of advice I have received here. I'm writing to ask for help in terms of relieving stress for me and my preceptor. My preceptor is recently very stressed. Since she's very kind to me, I feel like I need to do sth to help her out. I've learned to be a little more helpful. I'm not extraordinary but I'm progressing. Not a whole lot but gradually, step by step. I'm thinking of giving her some comfort food (dark chocolate bars or some fruit bars) to help her ease her stress. What do you all think?

ENCOURAGEMENT...CNA before nursing school??
go for it. best would be a cna in the ER or float pool and in a hospital you'd like to work at as an rn later on.

Help - I am interested in nursing but worried about the level of respect
i went to nursing school and condemned myself for choosing it 'cuz i was superly worried about how people are going to trust me. i was beating myself up for going to nursing until i found this awesome ER nurse who taught me so much that i'm in love with nursing and i don't care about anything anymore. find a good mentor and reflect on the time you've spent with her. mentors are everywhere. they are very nice. whenever life hit you hard, reflect on how those nurses react and how you admire them for their knowledge and experience. you'll make it. good luck.
