  3. Hi...I am relatively new to LTC and have done acute care for ~15 years. I am just loving the change! I have been assigned as case manager to one client and need help making a nursing care plan as this is an expectation for all case managers to complete. I am sure I haven't done one since nursing school! I don't even know where to start. My clients issues include occasional pain(relieved with T3s) , constipation (has colostomy...and T3s cause constipation) and insomnia. Can anyone help or point me in the right direction?

  5. and learned it the hard way! I was leaning over the end of someone's bed yesterday trimming some pretty skanky toenails.....one of them flew up and landed right in my open mouth!! Yuck yuck yuck! Can you gargle with the antiseptic handwash???? Note to self.....wear a mask AND close your mouth when trimming toenails in the future! :barf01:

  7. A few questions...My gran had a NSTEMI on Sunday, and now I find myself asking stupid questions that I know the answers to. -What should the ideal INR be after a NSTEMI? hers is 2.0 on Warf. (she was on it when she had the MI, so I am thinking it should be higher) and they do not have her on lovenox or heparin. She has also been on Plavix. -once you have done your initial 3 trops (we do then q8hx3) do you continue to check trop OD ? Thanks...

  9. What type of report does you unit do? We currently do taped report on night shift for day shift, and day shift does written report for oncoming night shift. So 2 different types of report. Its a busy general med unit.

  12. Cats from Heaven A pastor had a kitten that climbed up a tree in his backyard but then was afraid to come down. The pastor coaxed, offered warm milk, etc. The kitty would not come down. The tree was not sturdy enough to climb, so the pastor decided that if he tied a rope to his car and drove away so that the tree bent down, he could then reach up and get the kitten. He did all this, checking his progress in the car frequently, then figured if he went just a little bit further, the tree would be bent sufficiently for him to reach the kitten. But as he moved a little further forward....the rope broke. The tree went "boing!" and the kitten instantly sailed through the air-out of sight. The pastor felt terrible. He walked all over the neighborhood asking people if they'd seen a little kitten. No. Nobody had seen a stray kitten. So he prayed, "Lord, I just commit this kitten to Your keeping," and went on about his business. A few days later he was at the grocery store, and met one of his church members. He happened to look into her shopping cart and was amazed to see cat food. Now this woman was a cat hater and everyone knew it, so he asked her, "Why are you buying cat food when you hate cats so much?" She replied, "You won't believe this," and told him how her little girl had been begging her for a cat, but she kept refusing. Then a few days before, the child had begged again, so the mom finally told her little girl, "Well, if God gives you a cat, I'll let you keep it." She told the pastor, "I watched my child go out in the yard, get on her knees, and ask God for a cat. And really, Pastor, you won't believe this, but I saw it with my own eyes. A kitten suddenly came flying out of the blue sky, with its paws spread out, and landed right in front of her."
  13. Super glue?? No idea...haven't worn one since nsg school...and had a hard time keeping it on then. Maybe they should have a chin strap...like a birthday hat!LOL
  14. Yup...we do it all the time when necessary. As long as they are not diabetic.
