Nurse for the Railroad
I am currently in the process of hiring for a railroad. I'm not quiet sure what all is involved, but they look for nurses that are strong clinically and prefer acute ER/ICU experiences. My assumption on that is there's a sense of autonomy that has to be used in this position. I'm quite excited at the prospect. I've heard the money is great, and benefits are awesome!

International Nursing
I know this has been discussed so forgive me. i'm looking to do something non-US related. I have a friend whom i've lost contact with who did a 6 week job in Viet Nam and talked about how enlightening it was. I'm at a crossroads in my professional life and wondering if i do something like this it may help me decide where i'm supposed to go and what i'm supposed to do next. So i'm wondering, what agencies are out there? So far i've found out about Peyton Ogrady. But who else? or what organizations could i get in touch with to talk about nursing in other countries? Any thoughts or help are appreciated! :bowingpur

I disagree. i worked for Exempla (St. Joes, Lutheran, etc..) and they are not unionized. Not sure about VA.

Suicidal patients who are discharged from the ER
All of our patients have to have a psych eval, then the LSW deems whether or not they are a danger to themselves. Many times, people have a feeling of hopelessness or despair and state they want to commit suicide, however when discussing their feelings and given other options they realize it was just a fleeting moment type of feeling. The real issue is if they have a plan or not, etc.... So yes we have discharged many patients from the ER who stated on their arrival they were suicidal.

Alternative Loans - here's what I've found
I'm reading thru all of this and need to know, which one of these loans would be good for my husband. Our credit is not so great, but we are working on it. So to put it bluntly, who will do loans other than Stafford for him without good credit??

I have it too and saw my doc last month, not a podi, but my PCP. He put me on NSAIDS (Naproxen 500mg bid) and gave me some stretches to do and said if in a month its not better to come back and he'll inject me. Well its better but not great so i'm going back for injection next week. If you can't get in to the podi quickly, go to your PCP. They can handle it well too!!

It amazes me that people who don't consistently work night shifts think this is a hoax or for money. I went ot my dr crying begging for help because i'm like a zombie after about 6 hours, and then can't sleep!!! I have to do a sleep study, THEN he said we might try Provigil. I think if it works for you then GREAT. But for people who are insomniacs and can't function this is a great alternative!

We have Kaiser, and i pay around 350 a month for health/dental/vision and life. Its too pricey IMO.

personal awareness won't do anything to change it. If there's no one to watch your patients because they are equally or more busy than you are, then there just isn't anyone. Not to mention if i am working 12 hours, then i have to MAKE myself be on top of things for 12 hours. How many ER docs do you see that are well rested and fresh? None i know of. Our docs work super hard for long hours and they are totally on top of their game. This was just a venting post. I wouldn't change it for anything, i love the ER. I do not however love it when i'm worked like i was last night. But what are you going to do?

I swear when i started in the ED, i NEVER thought i would have to work as hard as i do. I'm sitting here, my feet are THROBBING. I work 11a-11p, and i didn't get lunch until 930pm. AND it was a 15 minute one to boot. One bathroom break. And i had an average of 5-6 patients ALL night, some very critical, some not. No wonder burnout is a major problem for ER. I HURT ALL OVER!!!! ok am over it. i'll shut up now. :rotfl:

Free-standing ER
THere's a new one here in Denver and so far the patients and the staff LOVE it. But i'm not sure how it all works just yet. Its a Healthone system here.

Hospital recommendations/advice-Denver area?
I work for Exempla Lutheran in the ER. We are now officially the busiest ER in the city according to the latest stats. We are a 54 bed unit, non trauma designated but working on our Level I certification. We are busy as hell and its hard work most days but the staff is overall wonderful. OUr average patient ratios is 5:1, unless they are critical. http://www.exempla.com and there are 3 hospitals in our system. HTH!

I work in Colorado at the busiest ER in the city. We take on average 4-6 patients a piece at a time and that's a LOT most days. I can't imagine where you work that requires that?!? OMG that's awful. I'd jump ship tomorrow if i were you. BUT the question too is is it acute patients or Urgent Care/Clinic stuff? Cuz our clinic side there's only one RN and 2 techs, sometimes a paramedic. BUT they are SUPER busy over there and only the RN can assess so that RN can sometimes really get hammered away with tons of assessments.

what treatment do patients who present with migraines......
Havne't read all the responses but we do: Benadry Pepcid Compazine or Zofran Toradol And then top it off with Dilaudid or Morphine occassionally. That and a 2 liter bolus of NS. If that doesn't work then a caffeine infusion.

propofol
we use it quite a bit in our ER. about 50/50. I agree with whoever said the patients seem to recover quicker with fewer side effects. It just depends on the patient, tolerance, size, injury, etc..etc...i've pushed it several times and never had a problem with it. however i've not ever done it without a MD at the bedside. Why would you ever push anything for conscious sedation without a doc at the bedside? And for that matter, we use a CS room where there's intubation equipment/BVM, etc...on hand. I don't see what the big deal is to be honest. If its legal in your state according to the BON, and you read up on the drug and have a doc in the room, then i'm ok to give it.

what do you do about childcare?
I had to have a family member watch them temporarily. Now i have a private nanny/sitter who comes to my home. i have 3 kids 5 and under, so i KNEW i'd be paying an arm and a leg. So she comes here and i do it from home. I advertised on craigslist.com for her and she's worked out GREAT!:wink2:

Its either in January or February and my DH wants us to go. But he didn't get all the details. Does anyone have ANY idea of anything going on? lol I know its a long shot, but hey you all know EVERYTHING here! Latex Allergy
2 words for you. NITRILE GLOVES. They are my SAVIOUR. I prefer the Kimberly Clark ones, but will take the med tech ones. I'm allergic to latex and vinyl. These are special ordered by my boss and work WONDERS. I really love the purple ones, but the blue ones work well too. They just fit a little looser than the purple ones. Give them a try, i hope it works for yoU!!!

Shirts, accessories, etc...
For home use, not work. i'd like something simple for work that has the ED or whatever on it.
