NCLEX question
I called the Board of Nsg after I failed to receive my ATT in due time, they emailed it to me the very next day. I took the Kaplan course ( 4 days ) It helped me a lot. I studied daily until I took the test. The test stopped at 75 questions and I came out feeling for sure that I had failed, but I passed, I got my license in the mail the next morning after testing. Good luck, and study, study, study, just keep doing all the practice tests over and over again.

State Mental Hospitals
I will be starting to work in a state run behavioral/nursing home type setting soon. Some of the pts have been in this facility for many years(mental retardation, Down's syndrome) and regular nursing home pts. Do any of you work in a similar setting? There will be 3 LPNs and 10 health service workers working on 4 units (total = 100 pts) I will be the only RN on nights. Any insight?

Sickle Cell Crisis question
I just had a SS pt last Tues. evening. She was in severe pain. She was on morphine 2mg q 30 min. basal rate PCA, then she could get .5mg q 10 min. by pushing the button. She was getting NS at 200/hr, and was on O2 at 2L/nc. Also she got 3 tylenol q 4 hrs for temp/pain.This is the first pt we've had on our unit for a long time. I've only been there 3 months, but the RNs who have been there for years couldn't remember ever having a SS pt. (I work in rural WV).

How often do surgeons order a basal rate for PCA?
I had a pt last night in with sickle cell crisis, she was getting PCA morphine with basal rate of 2mg per hour, plus .5 q 30 minutes as needed. She also got 3 tylenol q 4 hours prn. This did not help her pain that much. She was my first pt with sickle cell crisis. Her temp went up to 102, her arms and legs were aching. She had an IV going at 200/hr. I really felt sorry for her.

:imbar We did not learn I.V. sticks in nursing school. I need some advice,please. I am really bad at it. Any ideas, websites, or anything that will help, I will surely appreciate. My nurse manager tells me not to worry and that it takes practice, but I am worried. Thanks. :imbar

Saw my first code today
Vhope, That is so sad. Even though you don't know them personally, you can't help but think things like you said (not seeing snow anymore, seeing his kid's pictures...). Try not to dwell on it. {I had a nephew who committed suicide and it has ruined my brother's life, he is still grieving after 4 years. He is disabled d/t depression, dysfunctional grieving,etc.} The family of this poor man you are talking about needs our prayers. Take care. Dixie
