Likely depends on the state you are in, but I haven’t heard of challenging the exam. With my ADN program/state I was able to complete the PN-NCLEX after the first two semesters and passed without much difficulty. I don’t know if that helps.
Depends on you. Are you okay with following rigid rules/micromanagement? If not you can have great insurance, but burn yourself out at a job you don’t like.

Seems like the addendum would be more suited for a incident report rather than a patient chart. It was some form of management objectively commenting on their interpretation of the event as told by the nurse who witnessed it.
