Hi everyone! Im new here and Im glad that I found this site. Im applying for my NCLEX exam in IL but I have this problem on where to get the fingerprint card. Do I need to write them an email to request for the said form? A friend told me that you can buy it from review centers here at home but I think all they have here is a California form? Are they the same form? Please help. Thanks!

You should request by airmail to give you a copy of fingerprint card from the IL Dept. of professional Regulation.

request a fingerprint card by sending a letter through mail

or you can fax your request to:

it takes about 2-3 weeks before you recieve the card..
then go to the nearest NBI in your place..
send the card to IDENTIX and write a note on a piece of paper to forward the original fingerprint reciept to CONTINENTAL TESTING SERVICE.. you can also ask for your copy if you write it on the note...
