Here are some acute transfusion reactions. I'm not going to define them but I am going to list some so you can look them up.
Here are some delayed transfusion reactions.
Dehydration can be assessed by checking the skin tugor. Be sure to check the skin tugor of an elderly patient either on the forhead or sternum where the skin is not as elastic and loose. Also, an accurate intake and output should be performed to see what the patient is taking in and voiding. Specific gravity of urine can be performed to check how concentrated or diluted urine is. Usually with dehydration, the urine will be very concentrated. I hope this helps!

Just curious - have you read your textbooks or gone to Google or to a library? These are very basic topics, easily located.
What's going on with you that you came here instead of studying your books or getting to a library?
