Hello another hard exam today as usual. PLease answer the following questions and why you choose that answer.

1. A sevearly burned pt has been on Zantac for 5 days. What would the nurse assess?

a. bowel sounds

b. frequency of stools

c. percentage of meals eaten

2. client has been on Levaquin for UTI. After several doses he c/o vomiting and diarrhea. the nurse would health teach to?

a. take med in between meals

b. take with a full glass of water

c. discontinue the medication

3. MD orders an 10 year old with asthma two different kinds of inhalors (MDI). The nurse would instruct?

a. using a spacer helps to inhale the medication

b. inhale quick so the medication is better absorbed

Here are some hints:

1) What are GI side effects of Zantac? So based on the side effects, what would be best assessed?

2) Look in your drug handbook. What is the patient teaching for Levaquin?

3) What is the proper technique for using an MDI?

common side effects/adverse reaction to zantac is gi: constipation, diarrhea, nausea. add to that the pt is burned and looses a lot of hydration through the skin that is burned off. that could cause even more chance of constipation since the water is being lost through the skin.

2. pt is on levaquin for uti. after several doses pt c/o of vomiting and diarrhea. would would the nurse health teach?

a. take med in between meals

b. take med with a full glass of water

c. discontinue and call md

this one i am caught between two answers. i know that a would be wrong because an empty stomach with the med would make for more nausea and possible vomiting. they say to advise pt to drink at least 1500-2000 ml/day to prevent crystalluria. but that wouldn't stop the v/d. they also state to instruct pt to notify md immediatly if diarrhea w/fever occur. but the question doesn't indicate any fever. but diarrhea and vomiting are common side effects of levaquin.

3. md orders a 10 yr-old an (mdi) inhalor every 8 hours and another (mdi) inhalor four times a day. the nurse would instruct?

a. using a spacer helps administer the medication

b. inhale quickly so the medication is better absorbed

you are supposed to inhale deeply, not necessiarly quickly. a spacer helps the pt inhale the medication

hello nursing student here, i posted the following in student forum.....but no response. the following were test questions on my last exam, i still don't know the correct answer and don't see grade for a week, but these are driving me nuts. (daytonite.....where are you lol), please answer, thanks.

a. bowel sounds...5 days out..a severly burned pt likely isn't eating solid foods yet or producing stools. zantac is likely given to keep the stomach at rest during the initial stages of recovery.

b. frequencey of stools

c. percentage of meals eaten

2. pt is on levaquin for uti. after several doses pt c/o of vomiting and diarrhea. would would the nurse health teach?

a. take med in between meals

b. take med with a full glass.

c. discontinue and call md. n & v is a side effect for sure of many meds. a severly burned patient is at risk for hypovolemia anyways...any extra loss of fluids from n & v could present the start of a bigger problem. this side effect should be avoided. discontinue med and call doc for something else.

3. md orders a 10 yr-old an (mdi) inhalor every 8 hours and another (mdi) inhalor four times a day. the nurse would instruct?

a. using a spacer helps administer the medication. yup.that's the answer.

b. inhale quickly so the medication is better absorbed

1. I chose C b/c this med is more used for ulcers, gerd so I figured % of meals eaten would be correct. Did't think of frequency of stools d/t it it said just that, not that they had loose stools or diarrhea. And also, it said pt was s/p 5 days from burn injury and The whole fuid shift thing is in the emrgent/rescucitative phase which is the first 8-48 hours from injury.

2. I was stupped to put full glass of water or discontinue. Going with discontinue d/t the s/s, but when you look back to the question it states HEALTH TEACH, so then I changed answer to glass of water, thinking I'm wrong now.

3. I was always told not to read into the question, which I always do. So I chose B b/c the question doesn't say anything about a spacer....I was stupped on that one too.

P.S I have looked this up and still do not know the answers, that is why I'm asking here......for some feedback from professionals. Thanks.

Okay, I'm attempting to answer these without using any sort of reference. =) But I agree with the poster who said that you should easily be able to find the answers in the references that you, as a student, should readily have at your disposal. :D

Anyhoo...

1. I chose c. Nurse wants to assess the effectiveness of the Zantac as far as improving the patient's appetite/ability to eat.

2. I chose b. Taking the Levaquin correctly may prevent the patient from having the N/V, thus making it unneccessary to D/C the med.

3. I chose a. I do not believe that the child is supposed to inhale quickly, but deeply.

Ahhh... cool. Well, us professionals still forget our "stuff" from time to time, unfortunately. =) That's why you'll find a ton of reference books lying around any unit that you will work on. :D

Thanks for your tips though I have already looked them up in my books, but still not clear on the answers d/t what they actually asking in the question. Thanks anyway, just looking for some input here:typing.

As far as the spacer-- inhaling quickly is counterproductive. You are supposed to inhale slowly et deeply. Most spacers have some sort of mechanism that makes a humming sound if the pt is inhaling too quickly. I have never in the hospital setting seen an inhaler dispensed for a child without a spacer (sorry about the awkward sentence, you get my drift though).

b. frequencey of stools - i think the tip off here is "after 5 days" and "what would the nurse assess?" constipation is a side effect of zantac. even if the patient was on iv fluid or tpn he still makes stools or some size. five days of no stools is a problem.

c. percentage of meals eaten

2. pt is on levaquin for uti. after several doses pt c/o of vomiting and diarrhea. would would the nurse health teach?

a. take med in between meals

b. take med with a full glass of water

c. discontinue and call md - the vomiting is traumatic and a sign of gastric irritation; diarrhea is expected with antibiotics.

3. md orders a 10 yr-old an (mdi) inhalor every 8 hours and another (mdi) inhalor four times a day. the nurse would instruct?

a. using a spacer helps administer the medication - "mdi" stands for "metered dose inhaler" and only delivers the prescribed dose of a medication. a spacer is attached to the inhaler to help assist the patient in getting the dose of the medication as deeply into the lungs as possible: http://www.asthma.com/inhalers_vs_spacers.html and http://www.familyallergy.com/therapy/spacers.asp spacers allow the patient to breathe the aerosol in slowly and more deeply.
