Does anyone have nurse passes that includes some Covid symptoms?

I did away with passes last year since "sick" kids weren't supposed to come into the health office per our state rules. They were to be assessed in a "Medical Waiting Room" and if needing to be sent home, would be isolated in there, too, while waiting for pick up. Teachers/Staff need to call me before sending a student down. This works for me only because I'm in a smaller elementary school. I don't know what that would look like in a school with 500+ kids who are free to roam...

This is what I use and have been using for a few years. It covers the basics and they can write in the rest

We use a Google Form e-pass. The teacher fills it out, there are check boxes for why they are being sent and one "free space" to write in some details, and then the nurse gets an email notification. The nurse calls into the classroom when it's clear to send the kid. If it is an emergency - asthma, bee sting, etc. - the teacher can call for the nurse to come to the classroom or to send the kid to the clinic.
