Hello, I am currently undertaking a research project which entails teaching older adults to self medicate safely. I am looking for examples of medication schemas i.e. an easy to read list of the patients medications with appropriate instructions / pictures etc. which acts as an aid to memory. I was wondering if anyone has an example / sample of one....I am running out of time and just can't find one anywhere...thanks x;)

I made one for my job that is working really well for my home care pts. I take the actual pill and tape it in the left column. In the following columns are the name and dosage, what it's for, how many to take for each dose, how many times a day, and when to take it.

Thanks, Les. It was a lifesaver for one pt's wife, she was going nuts trying to figure out what meds were taken when after he had his stroke. When he stroked again she took it to the ER with her and the doc told her he wished all his pts had one of those.
