Medication forms.

We have been told that we need parent permission for all of our stock meds that are standing orders from our school doc. These are things like Bactine, artificial tears, A&D ointment, cough drops, TUMS and the like. Can anyone show me a type of form they use for something like this? I have well over 1,000 kids and I can't imagine getting many of these forms back and I also can't imagine having to refuse a kid contact lens solution because parent hast signed. Help!

Does your school do Health Information forms? i.e. registration forms that list medical conditions the student may have? If so, add a line at the bottom of the form with a yes or no check box saying the parent gives permission for first aid OTC meds. You can also include a line that says EXCEPT the following and have the parent write no Bactine/A&D ointment. So you will assume everything else is fair game.

That would work for the incoming kiddos but I need to get the permission district wide and also, would I need to obtain this permission yearly and if anyone does this, how do you keep track of who is/isn't allowed to use stuff? I my high school, I have a little table that is more or less "self serve" they can mostly take care of their own contact lens, dry eyes and paper cuts themselves. I'm there, but I don't oversee every tiny need.

I do this as well. I add a line that states "The school nurse may use first aid treatments, including topical ones, to treat allergic rashes, insect bites, toothaches, minor burns, dry/itchy eyes unless otherwise indicated by parent/guardian." If parent signs, I'm good to go and follow my standing orders.

But I will admit I do not monitor requests for saline rinse for contacts, I will give some in a small medication cup upon request. I also don't leave anything out aside for cups (water is from the fountain) and hand santizer (yes, please use it MS/HS kids, I beg you!) because it would be abused :).

We use an annual health update form with permission to contact MD re immunizations, physicals and OTC, a screen for more info re Health insurance and it is also the Emergency health form in the event they need EMS. They go out with the first day of school info and are expected back that week, that said I just got my last one yesterday!!!:banghead:

This is why I wish our registration would go online! It's crazy the papers that all these families need to bring in!! And online it wouldn't let you complete registration until EVERYTHING is filled out. So I wouldn't get half finished health histories or maybe a parent couldn't finish registration until a physical is turned in..... #pipedreams

That calls for a celebration! I am poking again for many missing forms - I also use them in case of EMS transport.

With contact solution look at the bottle. Some of that stuff is glorified purified water with no medication and therefore not medicated so in my last District students were allowed to carry it without a med form. Eye drops or medicated vs. eye rinse which is not.

For the forms, you could include that the parents permission is good for the entire stay within that school unless the parents fills out a new form or sends a written letter.

Also, don't forget to include that Emergency care always succeeds this! Something to that affect.

As far as the contact lens solution, my state (Pennsylvania) wants "doses" counted on the annual report.
