please email me or let me know your email... you just need to save it on 2003 excel to ensure that you have the format correct... as you add months and information into it, it will automatically calculate the number of infections and will do the sorting (as long as you pick what you want) and will also produce the graphs you need as it is entered.

Don't you get a monthly report from your lab. That's what I always used. The only info I had to look up was which ATB was used.

Looking for infection control logs/templates for LTC. If anyone has a template and would be willing to share it I would be so greatfull. The one we have is hard to get all the data collected and ask for data that we don't even have and unable to get. Anything would help. Thanks in advance

in a word type format create the document that would best meet your needs and satisfy state requirements...... but if you are at a facility that has corporate systems, you may have no choice..... have you sought help from your regional or corporate nurse consultant ?
