hi, Iam arabic nurse from jordan . and I am new graduated fron nursing callage and i had a new work on hospital . there is a case i want to ask about it . this case was remove a kidney stone and the doctor ordor to give her insulin infusion in 500ml D5W,i want to tell me why ? i know that we give insulin infused on d5w for DKA but why we give it for kidney stone removal pt? l was asked her doctor but he give me a brief answer .

You did the right thing, but if you still don't understand why you are giving a medication to a patient, keep asking until you have a thorough understanding or look it up in a drug book or web reference at your hospital. There's not enough information on your original post to determine why D5W was indicated, but just to review, these are some uses for D5W:

*hydration

*provides calories in the form of a carbohydrate and spares the use of your muscle protein

*can make you diurese

*can lower pH

*correct hypoglycemia

Did the pt have an elevated serum potassium, related to renal issues?

Small amts of insulin helps shift potassium from blood back in the cells, and will temporarily treat high serum K+ - the D5W keeps the sugar from dropping while this is done.

Agree, this is reasonably good English and devoid of the all so common "OMG'"s, "like gawd's" and other slang phrases that are commonly used by native English speakers.
