All kidding aside, what information would you like to have that you don't have? Is that information necessary to safely give the drug? Does your P&P manual provide the missing information? We can help you, but your learning experience will be more helpful if you work it out yourself.

Also, compare the dosing information provided for the patient's weight. Do they match?

Sorry for incomplete post. The order was avastin infusion: dose 5mg/kg; calculated dose 400mg; diluent/route: in 100ml NS IV over 60-90 min. my concern is that calculated dose is much greater than order dose ( over 20% ). Should I back to MD or just do it?

Based on posting history, the OP has held an RN license for more than three years, so this might not necessarily be a homework question.

If that's the case, then the OP should either clarify with the ordering MD/pharmacy, starting with the ordering MD. If the OP is questioning this as a current order on this forum, it's inappropiate/too late. ANYTIME a nurse has a question re a dose, s/he should contact the ordering MD/pharmacy immediately. ​Or am I missing something here?

Since we are on the topic of medication questions: I actually had a final in my math of pharmacology course yesterday and one questions really had me thinking. I calculated the child's weight based dose as 130mg Q6 PO but the doctor ordered 125mg Q6 PO, I said that I would not give this medication (it was an antibiotic) as we might be underdosing the child and would call to clarify with the physician. Am I wrong, is a 5mg difference negligible?

It entirely depends on the med as to whether that difference is inappropriate or not. And in real life, if you aren't sure, you check with your pharmacy or doctor before potentially doing anything damaging.
