Getting licensed in another state after surrender

I am currently licensed in NY and have been in recovery for 3 years. I surrendered my license due to diversion 3 years ago and completed all the requirements(2 year monitoring program with PAP). I have been discharged from PAP and have not had any disiplinary action. I am making a move to Florida and applying for license by endorsement. The application asks if you have surrendered in the past 5 years. I am hesitant to answer yes because I don't want it to affect my application. By the state of ny I have completed everything. What do I do? If I answer no, what can happen?

Your best bet is to consult a lawyer with experience in dealing with licensure issues in Florida. Good luck.

I too am up for reinstatement very soon and I will be moving to California in 10 months. My California application for endorcement also has a question relating to prior convictions, my demise was a DUI 3 years ago I too thought about no disclosing but really all they want to see is what you've done to correct the situation.i do not want to make a fresh start with not be truthful.. NEVER LIE! Never ever not tell them what you have been through. The BON really frowns on this.

My state only accepts endorsements for current & uncumbered licenses. Each BON has their own rules, but I think this is fairly common. If this is the case, OP would need to get the original license reinstated before trying to license in another state.

Check out the breEZe system, this is what many of the states have gone to for license verification and includes info from other states. This is the system that CA uses.

Information about nursing licenses is public record. If you answer no and the Florida board finds out the truth, my best guess is they would deny you a license.

I sat in a couple hearings in nursing school where nurses were brought before the board for not checking that box. I know each state is different but I would make sure I was fully informed about that states laws before you did it.
