Oh well it sounds like judging. You could of told me to look back and study assessments more in depth. Sorry I cant speak well english is my second language. She did tell me to focus on the diagnosis while doing head to toe. Esld I looked at her skin.. mouth gums. I just didnt lift up her shirt and look at her stomach. Although I did ask her if she felt pain or discomfort in that area. Noted color of wastes and strict I & o. What else am I missing? I would like some good feedback.. as of now I am investing my time looking at every diagnosis in book. Writing notes on assessments and primary diagnosis. Risk factors and all. At least her feedback was amazing other than she didnt give me a chance to prove I took her seriously. Told me I could come back but would in no way pass.

Did you listen to heart, lungs and bowels during your head to toe? Check for edema throughout. Check pulses? Why didn't you examine her abdomen?

I dk I think I just thought bowel sounds and asking about pain fullness discomfort was enough.

You need to listen to BS in all 4 quadrants, describe appearance of abdomen (soft, firm, distended, flat, rounded, bloated, tender/no tender, s/s pain such as "facial grimace with palpation and rebound LRQ, abdomen soft, flat +BS x4Q"

Bs x 4 felt it.. soft no firmness. No tenderness. No pain on palpation it was rounded but not like fluid full like ussual esld ascites.. she wasnt bloated didnt know about rebound on lrq

We were taught to observe, auscultate, and palpate the 4 quadrants of the abdomen, in that order. So, you had a pt diagnosed with abdominal ascites, but you didn't look at the abdomen?

I looked at it but did not lift the gown. I understand what you mean. Thank you for the help. We were also taught that way. Thinking clearly around someone who already failed you a week before is not so easy. Im a student.. I tryed my best but I was unable to meet up to her expectations. I will look back at my books and make sure I have my skills up to par. Past is over. Im going to work at it and get it right. I may also get an associates for science along the way.
