I am doing a careplan for school. I need to prioritize my nursing diagnosis. I am not sure which is most important.
Imbalanced nutrition: more than body requirements related to excessive intake in relation to metabolic need evidenced by his weigh of 358lbs.
Decreased cardiac output related to altered preload as evidenced by edema (swollen leg)
Ineffective breathing pattern related to obesity as evidenced by shortness of breath(while walking)
Activity intolerance related to sedentary lifestyle as evidenced by client's daily routine
I think the shortness of breath is the most urgent one (ABC). But hisweight is the underlying cause for all his problems. But the weight issue can not be quickly adressed, it takes time to lose weight. What do you think?

That's what I'm thinking too:
1. Ineffective breathing
2. Decreased cardiac output
3. Imbalanced nutrition
4. Activity intolerance

I was always taught, actual vs potential for dx's and then from there its always ABC's. So I echo what TRR8021 listed as most to least for priority.

Yep, ABCs, then Risks, also based on ABCs.
