What does this mean?

I would call them and ask them

It almost sounds like a marketing position to bring new residents to a facility after discharge from the hospital, like a case manager, doing assessments on prospective new residents to determine their d/c needs anad if that facility can meet those needs.

I was looking at jobs and you know how they kind of stay generic things. I was wondering if someone could give me a realistic explanation of what I would do in this job.

Performs the activities within the GHS Referral Center to grow the referral volume from outside health care facilities and physician offices. Accomplishes the responsibilities of this position by integrating and maximizing the work of various disciplines, resources, and services across departmental lines.
